Today, we would like to take some time to explain revision eye surgery by looking at photos of our patients.
For revision eye surgery, many patients visit us because their eyes look stuffy and they want them to be larger,
or because their existing double eyelids are too high and unnatural, so they want to lower the crease for a more natural look. These seem to be the most common cases.
We will take some time to explain by looking at photos of patients who fall into each of these categories.
CASE 1_Loose and Asymmetrical Double Eyelids
The first patient had their previous double eyelids loosen,
and wanted revision surgery because their eyes looked stuffy and were asymmetrical.






You can see that there are double eyelids on both eyes, but they look faint, and the right double eyelid is slightly larger.
Because there was thick fat above the eyes, we decided to remove the fat, raise the line to match the symmetry, and fix it using the incisional method.
Also, compared to the right side, the left side was slightly more blocked by an epicanthal fold, so we decided to release the epicanthal fold on the left side only.
The eyes were not very small overall, but due to the sunken eye structure,
they looked a bit stuffy. The patient wanted to enlarge the eyes as much as possible toward the outer and lower corners,
so we decided to perform a corner canthoplasty (lateral and lower canthoplasty) together to widen the outer and lower parts of the eyes.
PLAN
✅ Remove eyelid fat, match symmetry, raise the line, and fix with an incision
✅ Release the left epicanthal fold
✅ Widen the outer and lower corners of the eyes with corner canthoplasty
CASE 2_Lowering the Line (Sausage Eyelids)
The second patient is a male whose previous double eyelids were too large and thick,
giving him a greasy appearance, so he wanted to lower the line.






Both lines were quite large for a male, but the left line was relatively larger.
There was also a difference in the eye-opening muscle strength between the two sides.
The difference in eye-opening strength is determined by looking at the degree of exposure of the pupils.
If you look closely at the photo, the pupil of the right eye is clearly visible to the edge, whereas the pupil of the left eye feels a bit more covered.
If the eye-opening strength is different, even the same line can look different.
For example, if you relax your eyes and open them faintly, the double eyelid comes down and looks relatively larger,
but when you open your eyes wide with force, the double eyelid folds inward and the line looks smaller.
When the eye-opening strength differs between the left and right, the side that opens better will actually have a smaller-looking double eyelid,
while the side that opens less will have a larger-looking double eyelid.
Therefore, when performing double eyelid surgery, this must be balanced through a procedure called ptosis correction.
For this patient, we decided to lower the line and balance the eye-opening strength on both sides through ptosis correction.
What is a Double-line Incision?
When lowering the line, if there is enough extra skin and the distance between the desired lower line and the original line is close,
the surgery can be performed by excising all the skin in between and recreating a single-line scar.
Conversely, when there is no extra skin and the desired lower line is far from the original line,
it becomes difficult to excise the skin.
In such cases, a new incision is made at the desired lower line,
and the previous double eyelid is released only from the inside.
Doing this leaves scars in two places: the newly operated area and the previously operated area.
Because it results in two lines of scars, it is commonly referred to as a double-line incision.
This patient also needed to lower the line significantly and wanted the double eyelids to be covered a lot,
so a double-line incision was performed since sufficient extra skin was required.
The surgery was performed by making an incision at the new line, manipulating the eye-opening muscles to balance the strength on both sides,
and then fixing the line.
When a double-line incision is performed like this, multiple folds often reappear at the previous scar site,
so we utilized methods to prevent this from happening.
PLAN
✅ Lower the line and balance the eye-opening strength on both sides through ptosis correction
✅ Make a new incision at the desired lower line using the double-line incision method,
and release the previous double eyelid only from the inside
✅ Manipulate the eye-opening muscles to balance the eye-opening strength on both sides

Even though it is the day of the surgery, there is not much swelling because the line was lowered significantly.
(1 week post-op)
A little bruising is visible, but there isn't much swelling, right?
Even though it is an incisional double-line revision surgery like this,
you can see that there isn't much swelling because the line is lowered.
(2.5 months post-op)
The lowered line is well-maintained, giving a natural appearance.
When the eyes are closed or looking down, you can see that there are two lines of scars.
It is only visible when taking a close-up photo like this, and it is not noticeable in daily life.
It is still in the recovery process, so by the 6-month mark, the scars will become even less noticeable.
This time, we discussed common types of revision eye surgery,
taking the time to look at patient photos and talk about cases where the line is raised and recreated, and cases where the line is lowered.
