What is the cause of a drooping nasal tip (shape dissatisfaction)?




✅ Patients who experience a strong pulling force after surgery
✅ Those who smile a lot or touch their nose frequently
✅ When the nose is excessively elevated using only the patient's own cartilage
We discuss the shape extensively with the patient before the surgery,
but since we operate with a limited amount of material, it can sometimes be difficult to achieve the exact shape desired.
Also, for patients who experience a strong pulling force after surgery,
for example, those who smile a lot or touch their nose frequently,
the shape may change differently from the initial result. In some cases, patients themselves feel after the surgery
that it does not suit their face.
Does the nasal tip drop when smiling after rhinoplasty?


Although not many among my patients, it seems to be true.
Since cartilage is not as rigid as steel rebar,
it will be affected by the direction of the pull when smiling,
and if the nasolabial angle is pulled significantly and receives force when smiling,
there seems to be a tendency for the nasal tip to drop a little more.
Is it true that people with highly elastic skin are more prone to a drooping nasal tip?


Actually, those whose skin does not stretch well
seem to experience more drooping because the pulling force is stronger.
To prevent the nasal tip from drooping, is it better to raise it as much as possible during the primary surgery?


I believe it is important to perform the surgery in a way that minimizes the drooping of the nasal tip as much as possible.
If it is raised significantly during the primary surgery, some may not experience drooping and end up with an overly flashy look,
while others may droop moderately, and some may droop too much.
Therefore, it seems important to perform the surgery from the beginning to prevent drooping as much as possible.
Until when does the nasal tip drop after surgery?


✅ Generally noticeable within 2 weeks
In my observation, even comparing 2 weeks to 6 months, it's mostly just a slight reduction in swelling,
and it doesn't seem to drop significantly.
Of course, some patients may experience a bit more drooping depending on the individual,
but observing up to one or two months, it seems to barely change after that.
Is there a specific cause for a deviated nose after rhinoplasty?


Generally, since the nasal tip is elevated using a surgical method called septal extension graft, if the nasal septum was already deviated
and surgery is performed without correcting it, I believe it is bound to deviate.
Everyone's face is asymmetrical when looked at closely, including the shape of the cartilage, nostril shape, alar position, and the degree of nasal bone protrusion.
Perfect symmetry does not naturally exist.
However, after surgery, the nose becomes higher, making the same degree of asymmetry more noticeable.
We adjust what can be adjusted as much as possible during surgery,
but due to this inherent asymmetry, both sides cannot be 100% identical, which can sometimes make it look deviated.
If revision surgery is needed, is it better to do it after 6 months?


Because swelling can make asymmetry look more pronounced,
and even the swelling is not identical on both sides,
it is generally recommended to undergo surgery if a bothersome degree of asymmetry remains even after confirming that the swelling has subsided.
Also, since the shape may change slightly and look different over time,
if the part the patient is dissatisfied with is something that will be resolved to some extent as time passes,
we recommend waiting.
However, if the surgery resulted in something vastly different from the desired design, although very rare, we sometimes perform early correction.
If early correction is performed, within how many days after surgery can it be done?


✅ Possible between 2 to 3 weeks when major swelling has subsided!
Generally, you can consider the absolute maximum limit to be 3 weeks.
The 1st week is a period with a lot of swelling, so we observe until about the 2nd week,
and it is most commonly performed around the 2nd to 3rd week.
