Under-eye fat repositioning is a surgery that resolves bulging under-eye fat

or sunken under-eyes caused by a lack of fat.

Usually, when reaching the age of 30 to 40, the orbital septum structure surrounding the under-eye fat

weakens, causing the fat to push outward and bulge.

However, nowadays, many younger individuals are also visiting the clinic

due to dark circles and protruding under-eye fat.

Today, we would like to take some time to look into a real patient case of under-eye fat repositioning.

The photo above shows the before and after of a patient who underwent under-eye fat repositioning at our clinic.

Pre-operative Analysis

Before Surgery

This is a 48-year-old female patient, presenting a typical case for those considering under-eye surgery.

The area known as the tear trough ligament appears sunken,

and the under-eye fat bulges out above it.

The stark contrast between the hollowness and the bulging makes the dark circles look darker, giving a strong impression of fatigue.

The challenging aspects when considering surgery were:

1) The tear trough ligament was located at a significantly low position,

2) The characteristic of having a small maxilla (upper jawbone), with the left side being even smaller,

3) As a result, the under-eye area appeared sunken not only medially but also in a V-shape.

On the other hand, a positive aspect was that, compared to her age, she had few fine lines under her eyes,

and the orbicularis oculi muscle, which forms the aegyo-sal (under-eye charm), was providing relatively good support.

Diagnosis and Surgery

First, during the consultation with the patient, we had to decide whether to make an incision through the skin under the eyelashes (lower blepharoplasty) or

through the conjunctiva inside the eye.

Considering the lack of fine lines under the eyes and the adequate tone of the aegyo-sal mentioned during the analysis,

we determined that under-eye fat repositioning via a transconjunctival incision alone would yield satisfactory results and reduce the recovery period.

However, because the maxilla was quite small, the patient appeared sunken not only in the tear troughs but also in the front cheeks.

We believed that adding volume would improve the overall contour, so we recommended a small amount of front cheek filler.

After thoroughly dissecting the tear trough ligament area, we pulled out and dissected enough fat,

and fixed the fat using a fixation point 4-5mm further away from the trough ligament.

To ensure firm fixation and prevent recurrence, the orbital septum is also secured.

Areas with an excessive amount of fat were partially removed.

Afterward, we injected 0.8cc of filler into the left and 0.7cc into the right sunken front cheek areas.

For the filler, the patient wanted to reduce the sunken appearance rather than creating a voluminous look,

so we injected it naturally, taking the left-right asymmetry into account.

Post-operative Progress

✅ Improvement of bulging under-eyes

✅ Flattening of sunken tear troughs

✅ Improvement of sunken front cheeks

After the surgery, taping is applied with a gentle pressing sensation under the eyes.

There are no stitches to remove, and the patient visits the clinic on the 1st week when the swelling has subsided to some extent to check the surgical site.

In the photo taken 1 week post-op, you can see that the bulging areas have flattened and the sunken tear trough areas are clearly filled.

Because some swelling remains, the tear trough area might actually look slightly voluminous,

but the boundaries will become natural as time passes.

Additionally, the V-shaped hollowness and the slightly sagging impression it caused were naturally improved through the front cheek filler.

Compared to surgical methods that rely on filling with filler without properly dissecting the front cheek tear troughs,

thorough dissection and repositioning the fat underneath yield better results

and are superior in terms of long-term maintenance.

The patient above also did not receive any filler in the tear trough area,

yet the bulging and hollowness were well resolved.

Since it is not a surgery that causes severe bruising or long-lasting swelling,

I believe it does not pose a significant burden when deciding to undergo the procedure.
