Today, I would like to review a case of mini lifting, double chin liposuction, and thread lifting.


Before Surgery

The patient is a woman in her mid-50s who wanted to improve overall sagging in areas such as the front cheeks, side cheeks, and jawline.

Because she has consistently received treatments like thread lifting and skin boosters in the past,

her condition is relatively good for her age,

but it was inevitable that overall sagging made her nasolabial folds and marionette lines more prominent.

Since her facial volume is not heavy but there is skin laxity,

a lifting surgery involving incisions is appropriate for this situation.

As there is some fat accumulation visible in the double chin area,

we can consider double chin liposuction along with thread lifting and muscle tying.

Consultation and Surgical Process

For the midface, there was sagging and wrinkling extending from the nasolabial folds to the corners of the mouth,

so I recommended a mini lifting, specifically extending the incision to the front of the auricle

to maximize the dissection area and the amount of pulling.

For the side cheeks and V-line, it would be ideal to perform an incisional lifting (facelift) as well,

but since the patient prioritized minimal swelling and a quick recovery,

we created the line by performing only thread lifting with 3 threads on each side.

The double chin area was treated with double chin liposuction + thread lifting.

The liposuction addresses the central fat accumulation and enhances the overall submental line,

while the thread lifting holds the central area firmly inward

and has the effect of sweeping up the sagging skin.

Naturally, for patients in their mid-to-late 50s like the one above, performing a full facelift + neck lift

that dissects a wide area yields the best results

and has a longer maintenance period,

but minimizing the burden of surgery

while achieving satisfactory results through such combinations is the recent trend.

Post-operative Progress

For the mini lifting, the patient visits the clinic the day after surgery to remove the dressing on the temple area

and we check for any bleeding or fluid accumulation.

After confirming, the wound can be managed simply by applying ointment without a compression dressing,

so there is no disruption to daily life. Swelling mainly occurs around the temples where the SMAS layer is pulled and sutured,

which is an area mostly hidden by hair.

The area where double chin liposuction was performed is compressed with taping for at least 3 days to 1 week.

Even after that, wearing a facial compression garment when not going out

helps the swelling subside more quickly.

In this patient's case, she had to travel abroad, so she visited the clinic on the 3rd day

to remove the submental taping and have the wound checked once more.

You can see that the double chin threads are hooked strongly in the beginning.

Pulling it to the point where slight dimpling is visible ensures that when it settles after about 2 weeks,

it will show the appropriate effect.

The single stitch at the liposuction site under the ear is removed 1 week after surgery,

and the sutures and staples for the mini lifting are removed on the 10th day.

Looking at the 1-month photo, you can see that the midface and jawline are overall well-lifted and maintained,

and the double chin line is clearly defined without any feeling of sagging.

The scalp scar from the mini lifting is also not large

and is hidden by the hair. The scar in front of the auricle currently has some redness remaining,

but it will gradually fade over 6 months and become barely noticeable.
