Before Surgery

This patient is a woman in her 20s
who has monolids.
She visited our clinic because she was concerned about her stuffy-looking eyes.
Since this was her first eye surgery, she wanted the results to be as natural as possible,
hoping for a non-incisional method with an in-line to in-out fold.

Diagnosis and Surgical Method

In this patient's case, the eye-opening power was weak and the skin was thin,
but there was strong resistance, meaning the fold could potentially loosen.
However, because the patient preferred a non-incisional method
and wanted a natural-looking line,
we designed a crease that wasn't too high.
We also decided to add ptosis correction to reduce the chance of the fold loosening and to improve her eye-opening strength.
Additionally, the Mongolian fold was interfering with the formation of the front line,
causing it to cover the crease, so we decided to perform an epicanthoplasty as well.
Using the natural adhesion surgical method, we secured the fold with a 5-point continuous suture technique,
and then performed a separate 2-point ptosis correction to allow her to open her eyes clearly.
During the ptosis correction, because the opening strength of her left eye was weaker, we corrected it slightly more on that side.
For the epicanthoplasty, she didn't want the inner corners opened too much, so we kept it as natural as possible,
opening only a small amount while releasing the inner muscle to help form the line.
7 Days Post-Op

Because the amount of ptosis correction on the left side was relatively larger, the eye-opening strength appears significantly corrected,
and the line looks smaller. However, since non-incisional ptosis correction has a high tendency to regress slightly,
we expected the symmetry to balance out once the swelling subsides and the fold settles, so we decided to monitor her progress.
The epicanthoplasty area can develop a lot of scar tissue due to friction as the line forms.
Since this patient showed signs of this possibility, we planned treatments such as scar injections during her one-month follow-up.
2 Months Post-Op

As the swelling went down, the eye-opening strength and lines on both sides became symmetrical,
and the epicanthoplasty scars are barely visible, resulting in naturally defined eyes.
