Today, I would like to review a case involving incisional double eyelid surgery and epicanthoplasty.

Before Surgery

The patient is in her early 30s and visited us with concerns about her stuffy and heavy-looking eyes.
The skin above her eyes was significantly sagging and covering her eyelids,
and she had severely developed epicanthal folds.
Although the sagging skin covered a large portion of her pupils,
her actual eye-opening muscle strength was not weak, presenting what we call pseudoptosis.
Consultation and Surgical Process

The patient initially came in considering a non-incisional method for a natural-looking in-line double eyelid.
However, because her skin was quite saggy, if we set the line too low,
the overlapping skin from above would cause the fold to loosen quickly.
Conversely, if we set the line too high, the skin sagging below the line would be severe,
resulting in an unnatural and unappealing double eyelid shape.
Therefore, I recommended the incisional method,
and suggested that slightly raising the line to an in-out fold would be better than a low in-line.
Since her skin and muscles were somewhat thick, there would be some initial swelling,
but by slightly reinforcing the eye-opening strength using a 3-point fixation method when securing the line,
she could achieve clear, defined eyes without needing aggressive ptosis correction.
Regarding the epicanthoplasty, she had a lot of excess skin from the epicanthal folds,
which extended far downwards.
This area needed to be properly addressed to eliminate the stuffy appearance.
Since she didn't want the inner corners to be too sharp and preferred a natural look,
I performed the epicanthoplasty with a design that left a slight amount of tissue.
Because she wanted a natural line, the inner corner where the line begins
was designed with an incision that allowed it to be slightly covered.
The overall sagging and overlapping skin was planned for partial excision,
with a slightly larger amount removed from the left side.
Post-Operative Progress


On the 7th day when the stitches were removed, there was a fair amount of initial swelling,
and the epicanthoplasty looked somewhat under-corrected due to scar tissue.
However, in the 2-month progress photo, you can observe that her eyes look refreshing with an appropriate fold thickness and inner corner opening.
The wide distance between her eyes and the stuffy appearance caused by the developed epicanthal folds
have significantly improved, removing the blocked feeling
. Additionally, thanks to the appropriate double eyelid height and skin
excision, even without separate ptosis correction,
the pupils are adequately exposed, creating the effect of vertically enlarged eyes.
As the remaining minor swelling continues to subside over the next 3-4 months,
the fold will look slightly thinner, resulting in a natural yet larger eye shape.
