Today, I would like to review a case of non-implant rhinoplasty.

First, let's analyze the issues through the patient's pre-operative photos.

Pre-operative Analysis

The patient's nose was slightly short, but the nasal bridge itself had some height.

There was a slight dorsal hump, and the nasal tip was relatively droopy, making the nose appear long.

Additionally, the nasal tip was round and spread out,

and the retracted nasolabial angle made the nose look even more blunt and lacking in dimensionality.

The patient had an aversion to artificial implants,

so they wanted to improve the droopy nasal tip as naturally as possible using a non-implant method.

Diagnosis and Surgical Plan

Since the patient already had a decent amount of bridge height

and personally preferred a non-implant approach, we proceeded without artificial implants.

We decided to shave down the protruding dorsal hump, correct the bulbous tip,

shorten the appearance of the long nose caused by the droopy tip,

and bring out the nasolabial angle to add three-dimensional volume.

1 Week Post-Op

The dorsal hump was smoothly shaved down, and the nasal tip was elevated to match the bridge.

The step-off between the nasal tip and bridge was well-filled, leaving no visible irregularities,

and the nasolabial angle was brought forward to create a dimensional look. With the bulbous nose correction, the nasal tip became much slimmer.

2 Weeks Post-Op

Compared to the first week, the swelling has gone down further,

and as the residual swelling gradually subsides, the features look more defined.

How are a hooked nose and a bulbous nose corrected without implants?

The protruding hump is trimmed, and the drooping nasal tip is shortened and lifted for correction. According to the surgical cases in the main text, the procedure involved smoothly trimming the hump, raising the nasal tip to match the bridge, filling the gap between the bridge and the tip to eliminate contours, and refining the bulbous nose.

What are the effects of nasolabial angle correction?

It has the effect of improving a blunt-looking nose and lack of three-dimensionality. If the nasolabial angle is recessed, the nose can appear wider. By correcting the nasolabial angle to project outwards through surgery, a clearer and more refined nasal tip can be achieved.

When does swelling subside after non-implant rhinoplasty?

Significant swelling subsides from the first week after surgery, and by the second week, residual swelling decreases, and the shape becomes clearer. In the 1-week post-op progress in the main text, one can see the nasal tip becoming slender and the gap filled, and by the 2-week mark, residual swelling further decreases compared to the first week, making the nose line even more natural and defined.
