Today, we will review a case of mini lifting and V-line lifting.

Pre-operative Analysis
[Before Surgery]

This patient had previously undergone several procedures, including thread lifting and mini lifting.

She mentioned being more concerned about the nasolabial fold on the left side and the marionette lines on the right side.

In reality, the volume located just above the nasolabial fold (nasolabial fat pad) was heavier on the left side,

while on the right side, there was a feeling of the nasolabial fold extending down to the marionette lines.

Although the overall degree of sagging was not severe, because her cheekbones are on the smaller side,

the front cheek area lacked volume, making the midface appear longer even with a small amount of sagging.

Consultation and Surgical Process

While the lifting effect was important, she wanted to maximize the maintenance period, so we planned a mini lifting.

Uber's mini lifting is specialized for the midface, and the patient's requests were also focused on the midface rather than the lower face,

making her a suitable candidate for the surgery.

We designed the incision line solely inside the scalp and determined the direction that would provide the best pull.

However, due to a previous mini lifting, there was a wide scar formed inside the scalp,

so we considered a direction that could reduce the previous scarring.

The surgery involved making an incision inside the scalp to avoid any visible scarring,

while dissecting down to the temple and cheekbone areas enough to tie the SMAS layer.

By catching and tying the SMAS layer from the inside with threads at multiple points, an overall lifting effect of the midface can be achieved.

In cases where the volume just above the nasolabial fold (nasolabial fat pad) is heavy,

we sometimes add threads to strengthen the medial lifting of the midface.

After pulling up the inner tissues, a slight amount of skin remains on the scalp side.

Only the portion that spreads out without tension should be excised to prevent excessive scalp scarring,

thereby minimizing side effects such as hair loss.

After skin suturing, a compressive dressing is applied to prevent blood from pooling in the dissected area.

Post-operative Progress

In the case of mini lifting, since dissection is only performed in necessary areas, maintaining a simple compressive dressing for one day without a blood drain is sufficient.

From the next day, management is simple enough that you only need to apply ointment to the wound without any attached dressings.

Swelling is also not severe, similar to the level of a thread lift,

and you can expect it to be just slightly more pronounced in the temple area.

The stitches were removed on the 9th day,

and the front cheek area, which appeared sagging, was naturally pulled upward and smoothed out evenly,

filling in the sunken volume in the mid-cheek groove area.

As overall lifting occurred, you can see that the areas appearing as nasolabial folds and marionette lines have faded.

Looking at the 45-degree views on both sides, the area where the volume is most concentrated has moved upward,

achieving the effect of making the midface appear shorter.

The reason there is inevitably a difference in the maintenance period of the results is that,

when a thread lift is performed alone, adhesion only occurs in the areas where the threads are placed,

so the surface area is linearly thin, and once the threads dissolve, it inevitably reverts back.

However, mini lifting involves dissection from the scalp to the cheekbone area,

inducing a wide range of adhesion between surfaces, which we believe produces a semi-permanent effect in this area.

Since the surgical scar is also hidden solely inside the scalp, it will be covered by hair over time,

making it the optimal surgery for those who want a fast recovery without visible signs.
