Today, we will review a Mini Lifting case.

Let's analyze what the problem is through the patient's pre-operative photos.

​

Pre-operative Analysis

This patient is a female in her late 30s with previous thread lifting experience,

who mentioned that she was concerned because her front cheeks were not resolved during her previous thread lift.

Her front cheekbones are small, and her side cheekbones are of average width, while the midface and the fat tissue just above the nasolabial folds (nasolabial fat pad)

heavily overlap, making the nasolabial folds more prominent.

This is a typical appearance of a face beginning to sag. If facial contouring surgery had been performed,

this type of appearance could even appear in the mid-to-late 20s.

Since her skin is not on the thicker side, we determined that she could see sufficient results with a Mini Lifting.

Consultation and Surgery Process

The fat tissue above the nasolabial folds (nasolabial fat pad) has some volume,

so in this case, it is necessary to firmly grab and pull this tissue.

Since a Mini Lifting, which only involves incisions inside the scalp, cannot dissect down to this area,

1-2 threads need to be added to hold the inner tissue more strongly.

While pulling the patient's midface, we design the Mini Lifting so it can be pulled in the most efficient direction,

and we design the areas to add threads in a different direction so that each effect can be clearly seen.

The surgery involves making an incision of about 3-4cm inside the scalp,

and dissecting both above and below the SMAS layer so that the skin and the SMAS layer can be pulled and fixed individually.

In particular, if the SMAS layer is pulled well, the skin existing on top of it does not necessarily need to be pulled and sutured;

it is already well-lifted upwards. When the skin is raised without tension like this, scarring at the suture site is minimized.

After pulling at multiple points and suturing the SMAS layer, threads targeting the fat tissue above the nasolabial folds

(nasolabial fat pad) are added.

Finally, the remaining skin (scalp) is excised and sutured, followed by

a compressive dressing to prevent blood from pooling in the dissected area.

Post-operative Progress

In the case of Mini Lifting, since dissection is only performed in the necessary areas, a simple compressive dressing is maintained for just one day without the need for blood drains.

From the next day, management is simple enough that you only need to apply ointment to the wound area without any attached dressings.

The swelling in the midface is at the level of a thread lift, and the temple area where the SMAS layer is pulled and fixed

has some swelling and a feeling of hard clumping,

but it becomes natural after about 2 weeks.

In the 2-week photo, except for a small amount of yellow bruising remaining, there is almost no swelling,

and you can see an overall lifted appearance.

The midface, which looked saggy and heavy downwards, has spread evenly

to show a natural appearance, and even without fillers in the Indian band (mid-cheek groove) area,

some volume is filled in, making the boundary much less noticeable.

As such, since you can see a longer maintenance period and better effects than a simple thread lift,

while the recovery period is also short and scars can be hidden, if you are suitable for a Mini Lifting,

I think it is a highly satisfying surgery.
