A bulbous nose refers to a nose with a wide and blunt tip, often caused by thick skin.

A long nose is characterized by an elongated shape where the nasal tip droops downward, resembling an arrowhead,

or the drooping tip often makes a person look older than their actual age.

A hump nose has a protrusion in the middle of the nasal bridge with a downward-pointing tip, resembling a hawk's beak,

which can give off a strong, fierce, or aged impression.

A deviated nose can be a congenital or acquired condition, and in most cases, it is accompanied by functional issues

such as a deviated septum.

Today, we will look at a case involving a bulbous, long, hump, and deviated nose,

along with photos of an actual patient who underwent surgery at Uber Plastic Surgery.

This patient usually experienced discomfort while breathing due to her hump and deviated nose,

and aesthetically, her nose appeared long, so she wanted both cosmetic and functional rhinoplasty.

Pre-operative Analysis

Patient's Condition Before Surgery

✔ From the front, the nose is deviated to the right and appears S-shaped.

✔ From the side, a dorsal hump is present.

✔ The long nose gives a masculine impression.

✔ The skin on the nasal bridge is thin.

In this patient's case, the long nose took up a high proportion of the midface,

throwing off the facial balance. Although the nasal bridge was high, the convex dorsum gave a stubborn and fierce impression.

What kind of nose did the patient want before surgery?

She wanted a style similar to the celebrity Minji from NewJeans.

Considering that if we made the nose nearly straight and raised it significantly like Minji's style, it would look even longer and shift towards a more masculine image,

we decided to design it with a feel closer to a straight-curve (jik-ban) line.

Diagnosis and Surgery

For this case of a bulbous nose with a round tip + long nose + hump nose + deviated nose,

we harvested some of her own septal and ear cartilage.

Since the septum was deviated to the right, we corrected the deviation using spreader grafts,

and with the harvested septal cartilage, we performed a septal extension graft bilaterally on the existing septum to build a sturdy columella.

We tied the lower alar cartilages to the extended septum to elevate the nasal tip and resolve the bulbous shape,

and since the hump was severe, we performed an osteotomy to shave it down.

We decided to design and place a 3.5mm silicone implant tailored to fit her nasal bridge.

Post-operative Progress

The overall deviation was improved through osteotomy.

The dorsal hump has disappeared from both the 45-degree and lateral views,

and the nose has settled into a straight-curve (jik-ban) line, somewhere in the middle of a semi-curve, increasing the nasal height,

and the nasolabial angle has been improved, creating a more three-dimensional profile.

In this patient's case, the swelling subsided faster compared to others.

Although we need to monitor the final results up to 6 months post-surgery,

the patient is currently very satisfied, noting that her image has changed significantly,

and she mentioned that the functional aspects have also become much more comfortable.
