Today, I would like to review a case involving a bulbous nose, long nose, columella correction, nasolabial angle, and saddle nose.

[Pre-op to 3 Months Post-op]

Let's analyze the issues by looking at the patient's pre-operative photos.

Pre-operative Analysis

[Pre-op]

The patient's nose was slightly short,

and the nasolabial angle was retracted and low, resulting in a lack of three-dimensional volume in the midface.

Additionally, there was a slight dorsal hump and a saddle nose deformity with a depressed nasal bridge.

The patient wanted to correct the depressed areas and the dorsal hump

while improving the nasolabial angle with a natural line.

Diagnosis and Surgical Plan

Because the patient had a severely retracted nasolabial angle and insufficient septal cartilage,

we decided to build a strut using septal and donor rib cartilage. For the nasal tip, we planned to correct the bulbous shape and elevate it by adding ear cartilage.

For the nasal bridge, we planned to correct the dorsal hump and saddle nose, then elevate it using a silicone implant.

Although the glabella was not particularly high, the distance between the eyes was narrow.

To prevent the eyes from looking too close together, we decided not to elevate the glabella significantly,

and instead opted for a silicone implant that gradually increases in height toward the nasal tip.

-> Decided to provide a three-dimensional effect by elevating the nose and correcting the nasolabial angle.

1 Week Post-op

Although there is some swelling, the nasolabial angle has been corrected,

and the elevated nasal bridge has clearly brought out a three-dimensional volume.

2 Weeks Post-op

Compared to the first week, the swelling has subsided significantly, and the nasal tip has become much slimmer.

1 Month Post-op

It is well-maintained without major changes compared to the second week,

and as the minor residual swelling gradually subsides overall, a slimmer appearance is increasingly emerging.

3 Months Post-op

There are no major changes overall, but the swelling continues to subside nicely.

As the swelling around the nasal tip, alar, and nostrils goes down,

the nostrils are less visible, and the nasolabial angle feels more defined.

Pre-op / 1 Week / 2 Weeks / 1 Month / 3 Months Post-op

Overall, the nasolabial angle has been brought forward and the nasal bridge elevated, creating a three-dimensional effect,

and the bumpy nasal bridge has become smooth.

The blunt nasal tip has become slimmer, and the wide-looking areas have been beautifully refined into a more projected shape.

The slight upturned look up to the first week has settled as the swelling went down, maintaining a consistent shape from the second week onwards.

By the third month, the nostril visibility from the front has significantly decreased, and the nasal tip has been further refined to look slimmer.

Yes, simultaneous correction is possible. In cases like the one described, where a saddle nose with a sunken bridge and a protruding hump are present together, the hump and sunken areas can be corrected first, and then silicone implants or other prosthetics can be used to create a smooth and natural bridge line.

What materials are used if septal cartilage is insufficient?

Donor rib cartilage can be used together to build the nasal columella. If the patient's septum is insufficient to support the nasal tip, donor rib cartilage is added to create a strong columella, and then ear cartilage or similar materials are used to naturally correct the shape of the nasal tip.

What should be considered when performing rhinoplasty if the glabella is narrow?

It is important not to raise the glabella too much. For patients with a narrow distance between the eyes, excessively raising the glabella can make the eyes appear closer together, so it is advisable to use a silicone implant that does not raise the glabella much but gradually increases in height towards the nasal tip.

When does swelling after rhinoplasty become natural?

Typically, significant swelling subsides and begins to look natural from the second week after surgery. The nose may appear slightly upturned until the first week, but as it approaches one to three months, residual swelling in the nasal tip and alar subsides, and the nostrils visible from the front also decrease, settling into a slender shape.
