Today, I would like to review a case of double eyelid revision surgery.

A woman in her 40s, who had undergone incisional double eyelid surgery 30 years ago, visited our clinic.
She expressed dissatisfaction with her thick, unnatural double eyelids and the way the lines appeared ‘kinked’ rather than smooth.
Furthermore, she noticed that her eyelids had felt weaker over the past month, and even when trying to open her eyes naturally, they felt heavy and under-opened.
She had no history of Botox in the forehead area and did not show excessive use of the forehead muscles during the consultation.
Pre-operative Analysis



The current double eyelids are large and deep-set. The lines are angular and straight rather than being smooth curves, and the area below the line is bulging, giving a ‘sausage-like’ appearance.
There is skin sagging, and the distance between the eyes and eyebrows is relatively narrow.
The skin is thin, the eyes are slightly protruding, and looking at the exposure of the pupils, it is clear that the eyes are not fully opening, giving a sleepy impression.

The patient wanted to resolve the discomfort she felt when opening her eyes. She desired a clear and alert impression when opening her eyes naturally.
She also wondered if her unnatural double eyelids could be transformed into more natural, smooth, curved lines.
Diagnosis and Surgery

She had consulted at other plastic surgery clinics, some of which recommended a forehead lift.
However, since a forehead lift alone would not effectively improve the unnatural double eyelid lines, we decided to proceed with double eyelid revision and ptosis correction.
The patient also mentioned suffering from severe dry eye syndrome.
While she didn’t have difficulty closing her eyes during the exam, she was using her orbicularis oculi muscles excessively, resulting in many fine wrinkles around that area.
6 Days Post-operative


This is the appearance after stitch removal on the 6th day. Although swelling remains, the double eyelid lines have become smooth curves, and the eyes look larger and more refreshed than before.
The double eyelids have become naturally thinner, and the eye shape has changed to a more rounded form.
Despite the swelling, the overall impression is much softer.
Even when closing the eyes, there is no lag or pulling, showing a natural result.
2 Months Post-operative



This is the appearance after the swelling has subsided.
The thick, large, and deep double eyelids have been transformed into thin, natural lines.
The lines were lowered by changing the fixation point of the double eyelid without skin excision.
When closing the eyes, you can also see that the fine wrinkles caused by the previous over-reliance on the orbicularis oculi muscles have decreased.
