Today, I'd like to review a case of eye revision surgery.

A woman in her 40s, who had incisional double eyelid surgery 30 years ago, visited our clinic.

She expressed dissatisfaction with her thick, unnatural double eyelids and the angular appearance of her lines.

She also mentioned feeling a weakening in her ability to open her eyes over the past month,

and that even when she tried to open her eyes naturally, they felt less open than usual.

She had never received Botox on her face, and at the time of her visit, she was not excessively using her forehead muscles.

Her current double eyelids are large and deep. The line is not a smooth curve,

but rather an angular straight line, and the area below the line protrudes, giving a ‘sausage line’ appearance.

She has skin laxity, and the distance between her eyes and eyebrows is relatively close.

Her skin is thin, and her eyes are proptotic. Observing the size of her pupils, it's clear that her eyes appear less open, giving a sleepy impression.

The patient wanted to alleviate the discomfort she felt when opening her eyes after surgery.

She wished for a clearer, more alert impression when opening her eyes naturally.

She also wondered if her unnatural double eyelids could be transformed into a more natural and soft curved line.

Diagnosis and Surgery

She had consulted with other plastic surgery clinics, some of which recommended a forehead lift.

However, a forehead lift would not effectively improve the unnaturalness of her double eyelid line,

so we decided on double eyelid revision surgery and ptosis correction.

The patient also mentioned severe dry eyes.

During the examination, her eyes were not incompletely closed, but when she closed her eyes,

she used her orbicularis oculi muscles extensively, resulting in many fine wrinkles around the orbicularis oculi.

This is the appearance after suture removal on the 6th day post-op. Although there is still swelling,

both double eyelid lines are smooth curves, and her eyes appear larger and more refreshed than before.

The double eyelids have naturally thinned, and the eye shape has become more rounded.

Despite the swelling, she has a softer impression.

When her eyes are closed, they appear natural, without incomplete closure or pulled areas.

All the swelling has subsided.

The thick, large, and deep double eyelids have been transformed into thin, natural lines.

Without excising skin, the double eyelid line was lowered by changing the fixation point.

Even with her eyes closed, the fine wrinkles around her eyes, previously caused by orbicularis oculi muscle use, have reduced.
