Today, I would like to review a case of epicanthoplasty performed to correct inward-curling eyelashes.

Before Surgery

A female patient in her 20s visited us for a consultation regarding symptoms of her lower eyelashes poking her eyes.

She had a history of undergoing entropion surgery twice during her childhood,

so she had surgical scars on her lower eyelids.

However, even after becoming an adult, the inward-curling shape of her lower eyelids remained,

and she complained of ongoing discomfort from her eyelashes poking her eyes.

Eyelash poking in young patients is often caused by congenital entropion. ​

This refers to a condition where the edge of the eyelid (usually the lower eyelid) curls inward toward the eyeball, irritating the cornea.​

When this happens, patients often rub their eyes, experience excessive tearing, and may develop photophobia (light sensitivity).

In some cases, the eyelashes must be trimmed regularly.

Diagnosis and Surgery

This patient had a relatively narrow intercanthal distance of 34mm, so she was hesitant about undergoing epicanthoplasty.

She was worried that the surgery would make the distance between her eyes even narrower, causing her eyes to look too close together.

However, the amount of inner eye exposure during epicanthoplasty can be directly determined through the surgical design.

It is possible to excise the excess skin at the upper and lower edges of the eyelids without narrowing the distance between the eyes,

while simultaneously correcting the inward-curling eyelashes.​

5 Days Post-Op
[Before Surgery - 5 Days Post-Op]

She underwent rhinoplasty at the same time, so there is some swelling,

but she mentioned feeling much more comfortable as the eyelashes on both lower eyelids no longer irritate her corneas.

The distance between her eyes remains almost unchanged from before the surgery, so her eyes do not look too close together, nor does her impression appear harsh.

2 Months Post-Op

This is how she looks with all the swelling gone.

Because she had entropion surgery twice during her childhood, some scar marks remain.

However, the inward-curling of the lower eyelids toward the eyeballs appears to be significantly improved.

Entropion causes the edge of the eyelid to curl inward toward the eyeball, leading the eyelashes to poke the eye,

which can leave scratches on the cornea or even decrease vision.

Therefore, congenital entropion is often surgically treated during childhood.

Because the surgical area for entropion overlaps with the area for epicanthoplasty,

some people ask if they can improve their eyelash poking while getting epicanthoplasty at a plastic surgery clinic.

This is, of course, possible.

Epicanthoplasty itself has the effect of partially improving inward-curling eyelashes,

and even greater effects can be expected with additional procedures.
