Sunken upper eyelids are a common part of the aging process.

As we age, the eyelids gradually become more hollow.

With a significant loss of eyelid elasticity, drooping and hollowing of the upper eyelids occur.

When the upper eyelids become sunken, the eyes look hollowed out, and the double eyelid line

can form multiple folds, giving a tired appearance or externally

creating an older-looking impression.

Today, we will look at a real patient case who underwent upper eyelid fat grafting, along with their photos.

​

Upper Eyelid Fat Grafting

This patient is a woman in her 30s who had surgery using the non-incisional method about 10 years ago.

Over time, her double eyelids loosened and multiple folds formed,

so she visited our clinic complaining of sunken upper eyelids.

​

Pre-operative Analysis

​

Patient's condition before surgery

​

✅ Thin skin

✅ Sunken eyelids

✅ Multiple folds due to loosened double eyelids

​

She does not know whether fat was removed during her previous non-incisional surgery.

The patient inherently has very thin skin and a sunken eyelid structure,

and the left eye, where the non-incisional double eyelid has loosened, has developed multiple folds.

​

The eye shape the patient desires

The patient inquired if she needed to undergo double eyelid surgery again because her folds had loosened.

​

👉 Currently, the hollowing of the upper eyelids is severe,

so we informed her that if this is not resolved, there is a high chance the double eyelids will continue to loosen,

and we recommended upper eyelid fat grafting.

​

Diagnosis and Surgery

Since the double eyelids are not completely unraveled at present,

we could expect the lines to naturally reform on their own once volume is restored to the upper eyelids.

Therefore, while maintaining the existing double eyelid lines, we aimed to improve only the sunken areas of the upper eyelids

to eliminate discomfort when opening the eyes and create an eyelid shape with adequate volume and no multiple folds.

As a result, we proceeded with upper eyelid fat grafting surgery.

Post-operative Progress

Surgical Method

Under sleep anesthesia, the patient's own autologous fat was harvested

and separated into microfat suitable for grafting into the upper eyelids.

Afterward, local anesthesia was administered to the eyelid area,

and the sunken areas were filled with 1.5cc of microfat grafted into each side.

Since the double eyelid lines were not operated on separately, there is no swelling on the lines,

and swelling is only present in the areas where the upper eyelid fat grafting was performed.

After Surgery

The swelling has almost completely subsided.

The double eyelids maintain the shape from the previous surgery,

and the sunken areas of the upper eyelids have improved.

All surgical swelling has gone down, and the engrafted fat remains in the sunken areas of the upper eyelids, providing volume.

The patient mentioned that simply restoring volume to the upper eyelids makes it feel easier to open her eyes.

The multiple folds have disappeared, and a natural eyelid shape has been well-formed even when viewed from the side.

Because the surgery was performed by grafting microfat into the precise locations of the upper eyelids,

the grafted fat is not palpable, nor does it bulge out when closing the eyes,

maintaining a natural appearance.

💖 In cases where the double eyelid line forms somewhat faintly, 💖

simply filling the sunken eyelids with autologous fat grafting allows the double eyelids to form well.

Through upper eyelid fat grafting alone, you can simultaneously achieve natural double eyelids and upper eyelid volume.
