To improve a flat nose, short nose, or dorsal hump into an ideal shape,

rhinoplasty using implants is necessary.

Implants for rhinoplasty include artificial materials like silicone and Gore-Tex,

autologous tissues like ear cartilage, dermis, or autologous rib cartilage,

and various other materials such as donated cartilage or donated dermis.

Among them, autologous rib cartilage rhinoplasty uses material harvested from your own ribs.

Its advantage is strong durability, offering better retention than septal or ear cartilage, which is why it is becoming increasingly preferred.

Today, we will take a look at a real patient's case who underwent surgery using autologous rib cartilage, along with their photos.

This is a patient who had previously undergone surgery using silicone 10 years ago.

Pre-operative Analysis

Patient's Condition Before Surgery

✔ Silicone deviated to the right

✔ Drooping nasal tip

✔ Nasal congestion due to rhinitis

Before Surgery

This patient underwent rhinoplasty using silicone 10 years ago.

As a lot of time has passed, the current state showed the silicone deviated to the right,

making the nose look crooked when viewed from the front.

Also, the nasal tip was significantly drooping.

The patient had undergone two-jaw (orthognathic) surgery after their initial rhinoplasty,

and it is believed that the nasal septum was damaged during this time.

What kind of nose did the patient want before surgery?

First, they wanted to improve the drooping and slightly upturned feeling of the nasal tip when viewed from the side,

and desired a natural Jikban (straight-to-slight-curve) line.

They also wanted to improve their nasal congestion symptoms caused by rhinitis.

Diagnosis and Surgery

To perform surgery so that the nasal tip does not droop, the support structure is the most important.

Because the patient's nose skin was not very stretchable,

we harvested rib cartilage from the patient's body to build a sturdy pillar.

*Autologous rib cartilage is a safe and sturdy support that maintains its projection over time.

At the same time, we corrected the deviated silicone, improving the crooked nose.

We also performed an osteotomy simultaneously to improve the flat and wide-looking areas, refining it into a sleek nose.

Post-operative Progress

Compared to the pre-operative photos, the area where the silicone was deviated and crooked

has been straightened, and the crooked feeling has disappeared.

The nasal tip, which looked drooping compared to the bridge from the side and wide from the front,

has settled well into a Jikban (straight-to-slight-curve) line where the tip is slightly elevated compared to the bridge.

From the front, the tip has a defined point and is maintaining a well-projected look.

The photos above are from the 14th day and the 2nd month.

Even when comparing the 14-day and 2-month photos, the overall shape is well-maintained without any drooping or falling,

and as the swelling goes down overall, a more natural line is visible, and the nose is settling into a sleek shape.
