As we age, signs of aging naturally appear.

Many people experience increased facial wrinkles, deepening nasolabial folds due to sagging cheek fat,

and sagging around the mouth and jawline.

When the face sags overall like this,

the front cheek area can look flat and dull, giving an older appearance.

Today, we will review a real patient case who underwent Peach Lifting, along with their photos.

Pre-operative Analysis

Before Surgery

The patient has relatively small front cheekbones, with no volume above the mid-cheek groove boundary,

and a heavy-looking volume towards the bottom.

With this facial structure, even slight sagging of the front cheeks can appear quite severe.

Additionally, the nasolabial folds extend far down to the corners of the mouth,

and the front cheek tissue sags down to cover them, so we planned the lifting targeting this area.

Since the right side of the face is larger and has more volume, we determined the direction and strength of the lifting taking this into consideration.

Since the starting point of the nasolabial folds next to the nostrils is difficult to resolve with thread lifting alone,

we planned to use a small amount of filler for overall improvement of the nasolabial folds.

What kind of face did the patient want before surgery?

Diagnosis and Surgery

This is a typical case requiring Peach Up Lifting.

Using Mint Threads, we targeted the most sagging part of the front cheek volume

and pulled it up so that a substantial volume could rise near the mid-cheek groove.

Here, we used short Peach threads to shape the volume so it gathers well towards the center and upper part of the front cheeks.

By doing this, the volume covering the lower nasolabial folds is lifted, and the wrinkles become lighter.

Afterward, by using about 0.5cc of filler in the groove where the nasolabial folds begin to fill the volume,

the upper nasolabial folds also appear lighter, allowing for overall improvement in the shape of the nasolabial folds.

The lifting effect can be seen immediately after the surgery.

The area where the front cheek volume is most concentrated has shifted from the lower part to the mid-upper part,

and the long, descending nasolabial folds have significantly lightened.

As a result, when looking at the face, the gaze is drawn to the center rather than the lower part,

which also creates the effect of a shorter midface.

Also, through the appropriate direction of the threads, the side cheekbones and temple areas do not look wider,

and only the volume is lifted.

Since there was more volume on the right side, it may feel like the volume rose better and looks more plump immediately after.

As the swelling subsides and the threads settle over about 2 weeks,

the left-right symmetry will appropriately adjust to a more natural look.

If I were to recommend an additional procedure for this patient, since the anterior-medial cheekbones are small and lack volume,

performing a volume-filling procedure such as filler or Juvelook Volume would make them look a bit younger,

and I believe the hollowness in the tear trough area could also be alleviated.
