Today, we will review a case of a patient who previously received a Peach Lifting procedure at our clinic
and revisited us for a retreatment.

Pre-operative Analysis



Before the first procedure 6 months ago.



Before the retreatment.
The patient is a female in her early 20s who received the same thread lifting from me 6 months ago
and visited this time for a retreatment.
In fact, compared to typical cases, there is almost no sagging,
but her front cheekbones are small and her under-eye area is sunken, making her midface look long.
Her jawline has a slight accumulation of volume, and she wanted it to look a bit more refined.

Comparing the photo from her first procedure 6 months ago with the photo before today's procedure, the flat feeling in the midface is reduced,
and the jawline looks more refined.
However, you have to compare photos like this to see the slight difference.
Usually, the effect the patient actually feels is that from the lifted state,
it gradually drops, so they think it has returned to its original state.
That is why I do not tell patients that the duration will be more than 6 months when explaining the procedure.
Consultation and Procedure Process

For the front cheeks, Peach Up Lifting
was used to create volume while eliminating the feeling of the nasolabial folds being covered,
and for the jawline, we decided to use a minimum of 2 threads on each side just to slightly refine the line.
At this time, since there is not much sagging volume, there is a limit to the amount that can be pulled up.
Therefore, for a more definitive effect, I recommended a volume-filling procedure such as
fillers or Juvelook Volume for the inner front cheeks (the area under the eyes and front cheekbones),
but since she only wanted thread lifting, I explained this limitation and proceeded with the procedure.
Because there is some pain, I usually recommend sleep anesthesia,
and the patient also had it done under sleep anesthesia 6 months ago,
but this time she wanted local anesthesia.
When using local anesthesia, in addition to the entry point where the thread goes in,
anesthetic fluid is also injected along the path where the thread will travel.
Although the anesthesia process is somewhat uncomfortable,
the actual step of inserting the threads can be performed without significant pain.
The general direction in which the threads are inserted is predetermined,
but there are slight variations depending on the patient's face shape and degree of sagging,
and it also differs slightly depending on left-right asymmetry.
Also, because the insertion depth and the anchoring strength are different,
it must be entirely customized to each individual patient,
and therefore, I believe the results vary greatly depending on the skill of the performing doctor.
For the front cheeks, 2 Mint threads and 3 Peach threads are inserted on each side.
The Mint threads play the role of lifting the entire heavy volume,
and the Peach threads play the role of creating volume in the front cheeks
as if gently gathering the cheekbone and nasolabial fold areas with both fingers.
For the side cheeks, Mint threads or V-Seal threads are used
to target the area where volume gathers behind the marionette lines to revive the line.
After inserting and pulling the threads, I check if the tension is appropriate
and if there are any dimples (indentations), slightly loosening them if necessary.
Finally, with the head of the bed raised so the patient is sitting up, I check if there are any areas that are hooked too strongly and finish the procedure.
Post-operative Progress



During the period when the threads are fully settling in (about 2 weeks), thread lifting can easily come undone even with mild stimulation.
Especially if the tissue is very heavy, or conversely, too thin and weak, you must be more careful,
so I tell patients not to rub or pull their face for about 2 weeks.
Even so, the threads will loosen slightly during daily life
(when sleeping, making large facial expressions, etc.),
so taking this into account, the lifting is done with a slightly strong feeling right after the procedure.
Usually, it feels like it is pressed in or pinched with a finger,
and in most cases, it naturally loosens up as about 2 weeks pass.
Usually, these points are the sagging area right above the nasolabial folds and the sagging area right above the marionette lines.
If you look at the photo taken immediately after, the volume is well gathered toward the front cheeks, and there is a slight pinched feeling right above the nasolabial folds.
You can see that the outer eye area on the upper cheekbone is slightly swollen.
The more developed a person's cheekbones are, the more the cheekbones feel accentuated immediately after.
This is the area most vulnerable to swelling and bruising when performing thread lifting, so even if it feels a bit burdensome,
you do not need to worry because it will subside after about 2 weeks when the bruising and swelling resolve.



Since there is not much volume towards the jawline, pulling it strongly could make it look indented like a dimple,
so it was performed with a feeling of supporting it to prevent further sagging.
Later, in the photo taken at 1 month and 10 days, the excessive pinched feeling in the front cheeks has disappeared,
and appropriate lifting and front cheek volume effects are confirmed.
The midface looks shorter, and the jawline is refined together, giving the effect of a smaller face.
If additional changes are desired, I think creating volume in the inner midface with
under-eye fat repositioning and inner cheek fillers could result in a more vibrant face.
