Hello, this is Uber Plastic Surgery!
Today, together with Dr. Seo Seung-jo,
the topic for "Uber Tells You" is
the timing for rhinoplasty revision!
For those considering a revision after their initial nose surgery,
you might be wondering when the best time to do it is,
and we often receive questions regarding this timing.
With Dr. Seo Seung-jo of Uber Plastic Surgery,
let's dive into the details about this!

Generally, a rhinoplasty revision
should be performed 6 months to 1 year after the initial surgery.
This rule applies to almost all plastic surgeries.

However, depending on the cause for the revision,
the timing can vary.
It could be immediately after the surgery,
or at any time regardless of the initial surgery date,
and there is even a saying that nasal implants
must be replaced after 10 years.

First, the most common reason for rhinoplasty revision
is dissatisfaction with the shape, right?
In this case, it is best to undergo the revision
between 6 months and 1 year.

This is because during nose surgery,
after dissecting the cartilage structure layer and the skin layer of the nose,
implants and cartilage are manipulated and come into contact with the skin.
After that, the implants and cartilage need time
to safely adhere to the skin layer, right?
During this time, the swelling subsides
and the shape gradually changes,
and this process takes about 6 months to 1 year.

Then, why shouldn't you
have a revision surgery in the middle of this period?
It is due to the process of scar formation and maturation.
Surgical scars begin to form around 2 to 3 weeks
and then go through a process of gradually fading.
If a revision is performed during this time,
it is like hitting an already injured area again,
which can lead to contracture or deformation.

Second, within 1 to 2 weeks after surgery.
This is when the implant has shifted or the columella is deviated.
In such cases, it should be corrected quickly before scar tissue forms!
However, if there is still swelling from the first surgery,
rather than rushing into a revision,
it is advisable to make a decision after all the swelling has subsided.

Third, at any time.
Regardless of when the surgery was performed, there are cases
where a revision must be done as quickly as possible.

When you visit the clinic immediately after surgery (within a week) and are diagnosed,
if there are early signs of infection,
we can quickly irrigate and correct it
to help it heal cleanly.

However, if 2 to 3 weeks have passed since the surgery
and a full-blown infection has occurred,
there is no choice but to remove the implant.
Therefore, we recommend visiting the clinic within the 1st week after surgery if possible
to receive treatment.

Next is when the nose becomes upturned.
This happens when very fine, small inflammations occur repeatedly,
leading to a contracture phenomenon.
In this case, the silicone must inevitably be removed.

Next is when the tip of the nose turns red.
This is a symptom that appears when the silicone gradually shifts downward
or when the thickness of the skin covering the tip of the silicone
becomes less than 1 to 2 mm.
If it is the latter, where thin skin is covering the implant,
it wouldn't be surprising if the silicone protruded in just 1 to 2 weeks,
so it is best to perform a revision surgery quickly.

Lastly, some people say that after 10 years from the surgery,
a rhinoplasty revision must be done.
I do not believe this is true.
In the past, L-shaped silicone implants were widely used,
inserting silicone in an L-shape from the nasal tip to the nasolabial angle,
and as the nose could not withstand the silicone,
it led to the generalization that a revision is needed
after 10 years.

If the silicone and the shape of the nose are maintained normally,
there is no need to replace it every 10 years.
