Uber Plastic Surgery's Dr. Seo Seung-jo Explains! Tip Rhinoplasty: Is Tip Cartilage Tying Enough? From the exact concept of tip cartilage tying to whether surgery is possible, we explain it all!

Hello. This is Uber Plastic Surgery!
Today's topic is from our last 'Reading Rhinoplasty Comments' episode,
which took up the largest portion of questions,
and it is all about tip rhinoplasty!
Since so many of you were curious, we specially prepared
today's topic for 'Uber Explains'!
It is: "Can tip rhinoplasty be done
solely through tip cartilage tying?"

Dr. Seo Seung-jo will explain the concept of tip cartilage tying
and whether this surgery is possible for you,
so shall we go watch it right now?

There are three types of tip rhinoplasty.
Erecting a columellar strut,
tying the tip cartilage,
and tying the tip cartilage followed by a cartilage graft on top.
However, what many people ask about
is whether they can raise their nasal tip
simply through closed tip cartilage tying alone.

Before we find out if that's possible,
shall we look into the exact definition of tip cartilage tying?
The types of tip cartilage tying
are not limited to just one;
there are actually many different methods.

There is a method of tying the two cartilages together,
a method of tying each cartilage individually,
a method of connecting the two cartilages,
and a method of pushing protruding cartilage inward.
Additionally, the nasal tip can be raised or lowered.
Among these, the tip cartilage tying you generally think of
is the first method mentioned: bringing the two cartilages together and gathering them.

By using the method of bringing the two cartilages together
to perform tip cartilage tying,
the width of the nose is reduced as the cartilage is gathered,
and because it is raised while being gathered, the nasal tip also goes up.
This allows for both effects: raising and gathering the nasal tip.

However, there are cases where the effect of tip cartilage tying is less prominent.
For Asians, who relatively have thicker skin, the effect may be diminished.
Even if the tip cartilage is diligently tied and its shape changes,
if it is covered by thick skin, nothing will be visible,
and the surgical effect will decrease.

Therefore, if the skin is not too thick and is appropriately thin,
performing tip rhinoplasty solely with tip cartilage tying yields good results.
If the support of the alar cartilage crura is strong, tying it from the inside
allows the crura to support that force well.
So, if the columellar strength and the alar cartilage crura are somewhat strong,
tip rhinoplasty through tip cartilage tying is possible.

Just as there are cases that turn out well
with tip cartilage tying, there are also cases that can go wrong, right?
Tip cartilage tying essentially pinches the nose,
so if someone with very thin skin has their cartilage tied incorrectly,
it can result in an overly pinched nose.
Also, if the nasal bridge is originally low but the tip is reduced and raised,
only the tip will go up like Pinocchio, leading to unnatural results.

To succeed in tip rhinoplasty solely through tip cartilage tying,
there are many factors to consider.
However, if your skin is appropriately thin and soft,
the support of your alar cartilage crura is strong,
and your nasal tip is drooping,
Uber Plastic Surgery can create a beautiful nose
in the simplest way using only the tip cartilage tying method.

Whether you are a suitable candidate
for tip rhinoplasty using only tip cartilage tying
is difficult to judge on your own,
so if you are considering surgery,
we recommend getting a consultation with Dr. Seo Seung-jo at Uber Plastic Surgery!
