Located in the center of the face, the nose is one of the features that greatly influences first impressions,

and it is the second most frequently operated area after the eyes.

Because of this, the frequency of revision surgeries is also on the rise.

While some undergo revision surgery due to dissatisfaction with the shape or height,

there is an increasing number of cases where revision is needed due to capsular contracture—a condition where the nasal tip turns upward and shortens due to surgical failure, causing a deformity in the external appearance of the nose.

What causes a contracted nose?

When an implant, such as silicone used in rhinoplasty, enters our body, it becomes surrounded by a tissue called a capsule.

Normally, the implant remains stable within this capsule.

However, if micro-inflammation occurs repeatedly, the capsule becomes hard and shortens, pulling the overlying skin.

When the cartilage strut used in the surgery cannot withstand this pulling force, the nose may become upturned, pinched, show visible cartilage, or even deviate.

What are the symptoms of a contracted nose?

There are symptoms that can lead you to suspect a contracted nose is developing.

You may notice a foul odor from inside the nose, or a micro-inflammatory response may occur, causing a sensation of a foreign body.

Additionally, symptoms such as the nose swelling or turning red can occur when you are fatigued, such as after heavy drinking or working late.

These symptoms may repeatedly improve when you take medication or your condition gets better.

If acne or sebum-like breakouts repeatedly occur in the same area, you might also suspect the progression of a contracted nose.

if you observe the aforementioned shape changes—such as the nose turning upward, looking pinched, cartilage becoming visible, or the nose deviating—

compared to previous photos, it is advisable to suspect a contracted nose and seek a medical consultation.

What is the solution for a contracted nose?

It is crucial to thoroughly release the adhesions in the area causing the contracture and perform a wide dissection to allow the skin to stretch adequately.

Secondly, the supporting strut that acts as a pillar must be built more robustly than in a standard rhinoplasty.

★ For noses with severe contracture, the strut is built using autologous rib cartilage, which is the hardest cartilage in the body.

Furthermore, if the nose is elevated excessively, high pressure will build up on the nasal tip again, so the surgery must be performed within a safe limit without overdoing it.

In cases of such contracted noses, taking contracture prevention medication and receiving scar tissue injections post-surgery can also be helpful in preventing the contracture from recurring.
