Natural Adhesion (Dermal Adhesion) Double Eyelid Real Case Review - Correction of Unraveled Eyelids and Asymmetry

This is a natural adhesion (dermal adhesion) case review of a patient who had unraveled double eyelids and asymmetrical eye shapes. The surgery was performed using a non-incisional method with a semi-out line design to refine the crease and alleviate asymmetry. Through the 2-week and 3-month postoperative progress, you can see the crease stabilization and the improvement of bilateral balance.

Hello~

💎your beauty universe! This is Dr. Shim Jeong-hwan from Uber Plastic Surgery :)💎

Today, I would like to review a natural adhesion (dermal adhesion) case.

Before Surgery

The patient previously had double eyelids, but the lines were not stably maintained and had unraveled.

Additionally, due to facial asymmetry, the shapes of both eyes appeared different from each other.

The right eye had a longer horizontal length and a relatively flat shape,

while the left eye had a larger vertical height but a shorter horizontal width, giving it a rounder appearance.

✅ Existing double eyelid lines, but in an unraveled state

✅ Asymmetry present due to differences in the shape of both eyes

Diagnosis and Surgical Method

To more actively correct the asymmetry of both eyes,

methods such as incisional double eyelid surgery, ptosis correction, and excision of sagging skin

can enable more precise symmetry correction.

However, the patient did not want that extent of correction

and preferred a direction that naturally refined the lines while alleviating the asymmetry.

Accordingly, the following surgical plan was established.

✅ Proceed with non-incisional natural adhesion double eyelid surgery

✅ Ptosis correction not performed (considering the limitations of the correction effect with the non-incisional method)

✅ Approach focused on alleviating asymmetry through line correction

Furthermore, the design the patient desired was a semi-out line,

setting the goal for a line that does not go inside the epicanthal fold

but connects naturally towards the outside.

2 Weeks After Surgery

Initially, the double eyelids may appear somewhat deep and unnatural due to swelling,

but this is a normal progression commonly seen during the recovery process.

✅ Lines stably formed in a semi-out line shape

✅ The heights of the double eyelids on both sides are matched similarly

✅ Slight asymmetry due to the inherent difference in eye size partially remains

3 Months After Surgery

Looking at the 3-month progress photo, you can see that the swelling has sufficiently subsided,

and the lines have settled into a much softer appearance than in the early stages.

The semi-out line the patient wanted does not go inside the epicanthal fold at the front,

but connects naturally to the outside and is clearly formed.

The heights of the double eyelids on both sides are also balanced,

showing a much more alleviated asymmetry compared to before the surgery.

Although some structural differences due to the discrepancy in the horizontal and vertical proportions of the eyes remain,

the overall impression has been improved to a more refined eye shape

just through the line correction.

✅ Lines have settled much more naturally due to the reduction in swelling

✅ Improved harmony between both eyes, showing alleviated asymmetry

✅ Completion of double eyelids with an overall soft and natural impression

Before Surgery / 2 Weeks / 3 Months

Depending on the shape of the eyes and the degree of asymmetry,

the appropriate surgical method and design may vary.

If you have similar concerns, it is important to accurately analyze your current eye condition

and establish a plan in a direction that suits you.

If you need a more detailed consultation,

please feel free to inquire, and we will guide you according to your individual condition.

Frequently Asked Questions

What is the natural adhesion (dermal adhesion) surgery method?

In this case, it is a natural adhesion double eyelid surgery performed using a non-incisional method, which is introduced as a way to form a natural double eyelid line by refining the crease without incisions.

Can unraveled double eyelids also be corrected with natural adhesion?

In this case, you can see the result of re-refining the lines using the non-incisional natural adhesion method for a patient whose existing double eyelids had unraveled, settling into a stable semi-out line shape.

Can eye asymmetry be completely corrected with natural adhesion alone?

According to the post, asymmetry can be alleviated just by line correction, but structural differences such as the discrepancy in the horizontal and vertical proportions of the eyes may partially remain. For active correction, incisions and ptosis correction are considered together.

Was ptosis correction performed together in this case?

Considering the patient's preference and the limitations of correction with the non-incisional method, ptosis correction was not performed. The surgical plan was established to focus on line refinement and asymmetry alleviation through non-incisional natural adhesion.

What kind of shape is a semi-out line design?

It refers to a line that does not go inside the epicanthal fold but connects naturally to the outside. In this case, a clear yet natural semi-out line is confirmed to have formed at the 3-month mark.

How does the recovery progress after surgery?

Initially, the lines may appear deep and unnatural due to swelling, but this is a normal progression. By the 2nd week, the lines settle, and by the 3rd month, they stabilize into much softer and more natural lines due to the reduction in swelling.
