Today, I would like to review an incisional double eyelid surgery case.

Before Surgery

This patient was having eye surgery for the first time,

and overall, her upper eyelids were thick and accompanied by sagging skin.

In particular, the front epicanthal fold extended downwards, covering the area in a rounded shape,

creating a stuffy and sleepy-looking impression.

Additionally, she had a habit of using her forehead when opening her eyes, so her eyebrows were observed to be raised.

The patient wanted a clearly visible line like a semi-out fold or an out fold,

but her existing eye structure made it difficult to form naturally.

Diagnosis and Surgical Method

This was a case with sagging upper eyelid skin, thick eyelids with a lot of fat,

and strong epicanthal folds.

Because the non-incisional (suture) method has limitations under these conditions,

we proceeded with a complex correction based on incisional double eyelid surgery.

The surgery was performed as follows:

At 2 weeks post-op, some swelling still remains,

but you can see the line stably settling into place.

The area where the epicanthal fold was opened has improved from the previous rounded, covered feeling,

and a semi-out fold shape where the front line starts naturally is forming.

As it was performed with an incision, you can also see that the line is maintained clearly.

In addition, the thickness above the eyes has been greatly improved due to fat removal,

and the use of the forehead when opening the eyes has decreased due to the ptosis correction effect, with the eyebrow position naturally coming down.
