Many people ask, "Does facial contouring surgery make the midface shorter?"

But in reality, there is no surgery that actually reduces the facial area.

Anatomically speaking, there is no surgery that makes the midface shorter.

I will explain why that is the case in just a moment.

Why is a long midface considered undesirable?

Q. Why is a long midface considered undesirable?

Why do people care so much about their midface?

Many patients come to us saying, "My midface is too long, so my face looks old."

That means a long midface makes you look older.

The reason for this is that when we look at young children, almost none of them have a long midface.

Why does the midface grow as we age? There are air cavities like the maxillary sinus,

and these gradually get bigger.

Since young children have almost no air cavities, as they age, these cavities gradually expand,

and as they grow larger, some people's midfaces become progressively longer with age.

So, a long midface gives off the impression of looking older.

Where does the midface start and end?

First, before thinking about how to make the midface look shorter, we need to talk about where exactly the midface is.

Where do you think the midface area is?

1. Glabella to the nasal tip

2. Glabella to the alar base

Strictly speaking, according to textbooks, the midface area is from the glabella to the base of the alar (nostrils).

What surgeries make the midface shorter?

Since the general public perceives the midface as extending down to the philtrum,

it can be made to look shorter through philtrum reduction or orthognathic (two-jaw) surgery.

1. First, Philtrum Reduction

I think it is a very good surgery.

If the philtrum looks long, it can give a "grandmotherly" appearance, making you look much older, and the lips appear droopy,

giving a lack of vitality.

For those with a philtrum of 14mm or longer, just shortening the philtrum can make the impression look brighter.

However, the downside is scarring! If philtrum reduction is done through an incision above the upper lip, the scarring can be quite severe.

If done incorrectly, the lip line can appear white, leaving a flat impression, so these days, incisions above the lip

are rarely done, and subnasal incisions are much more common.

2. Second, Orthognathic Surgery

It can slightly shorten the maxillary bone area of the philtrum.

In this case, it does get shorter, but if done incorrectly, the skin may appear to sag,

creating an optical illusion that makes the philtrum look even longer.

Whether orthognathic surgery can make the midface shorter and whether it will be effective should be decided after a thorough consultation with a specialist.

Are there any other ways to reduce the midface?

Methods to make the midface look shorter, rather than actually making it shorter!

First, the most important thing is giving the face a three-dimensional effect. Even if it is long vertically,

if there is front-to-back dimensionality, it will look much shorter than you think.

Among those, the most important is the nose!

1. A Three-Dimensional Nose!

If the nasal tip is droopy, the center of the face will look very long.

So, rhinoplasty should be done in a way that slightly raises the nasal tip, in a way that doesn't make it look long,

and when created like that, the face can look a bit shorter.

2. Elasticity Lifting

If sagging areas are lifted, it looks shorter.

The midface doesn't actually get shorter, because the position of the alar base doesn't change.

However, because the tissues are lifted, it can look a bit shorter.

3. Fat Grafting

By adding volume to the front and back through fat grafting, it can look a little shorter.

For the upper area, this is possible through under-eye fat repositioning.

If the tear troughs are sunken and hollowed, and the under-eye fat protrudes, it gives the impression that the midface is long.

By making the under-eye fat less visible and emphasizing the volume of the front cheekbones, you can give the impression of a slightly shorter face.

Among these, I think I prefer the method of making the midface look shorter through rhinoplasty the most.

If the nose is overly upturned, the philtrum will look long and exposed, making the midface look long.

Usually, people think that having a curved line makes it look shorter.

But if a curved line is made incorrectly, the nose will look long.
