Because of this, many patients who visit our clinic come with the thought,
"I definitely need to get under-eye fat repositioning."
However, during the consultation, there are cases where we recommend a procedure called lower blepharoplasty instead,
and many people are unsure of how the two surgeries differ.
So today, we will take the time to look at these two surgeries that seem similar but are actually different,
specifically what under-eye fat repositioning and lower blepharoplasty are,
and what exactly the differences are between them.
What kind of surgery is under-eye fat repositioning?




Generally, when the fat under our eyes looks bulging and dark circles are severe,
we often consider under-eye fat repositioning surgery, right?
So first, it would be appropriate to understand why dark circles under the eyes occur.
There are roughly three main reasons.
1. When the color of the skin under the eyes itself is slightly darkly pigmented
In this case, rather than applying under-eye fat repositioning surgery, we can say that dermatological treatments,
such as pigment lasers or other procedures, are needed to resolve it.
2. When it is caused by the fat under the eyes
Simply put, it can be divided into cases where there is too little fat under the eyes, or cases where too much fat has accumulated.
The skin under the eyes is thinner than the skin on other parts of the face.
So, if there is too little fat under the eyes, it is resolved by performing micro-fat grafting and spreading it out well.
If there is too much under-eye fat, the fat bulges out and creates a shadow under the eyes,
making a person look tired and dark. This can be resolved with under-eye fat repositioning surgery,
which appropriately removes the fat and repositions it.
Under-eye fat repositioning is performed by flipping the lower eyelid and making an incision in the conjunctiva,
so it has the advantage of leaving no surgical scars on the skin.
What is lower blepharoplasty?



Now, there is one thing to consider here.
It is true that under-eye fat repositioning surgery can be performed regardless of age or gender
when there is an excessive accumulation of fat under the eyes.
However, if there is a decrease in the elasticity of the skin under the eyes, wrinkles, and accompanying skin sagging,
there are many cases where under-eye fat repositioning alone cannot resolve the issue.
Good results can only be achieved by excising some of the skin and pulling up the sagging muscles.
Lower blepharoplasty includes all these processes: excising the stretched skin, removing and repositioning the fat,
and firmly fixing the muscles that have lost elasticity to the periosteum.
However, since a skin incision under the eye must be made to perform this process,
stitches will be visible on the outside, and slight surgical scars may also be visible.
To summarize, you can think of lower blepharoplasty as a broader scope of surgery
that can remove wrinkles by organizing sagging skin under the eyes, along with under-eye fat repositioning and fat removal.
Which surgery is right for me?


Some of you may have already gotten the gist just from the previous explanation.
Although categorizing by age is a bit less accurate, if you are in your 20s or 30s and are worried about dark circles or bulging under the eyes, you will likely be able to achieve the desired effect sufficiently with just under-eye fat repositioning.
However, for those in their 40s and older, depending on their skin elasticity, degree of sagging, and degree of fat bulging, there may be cases where fat repositioning alone can resolve the issue, and cases where lower blepharoplasty is required.
Usually, when the under-eye area is sagging, the upper eyelid is also sagging in many cases, so there may be cases where upper and lower blepharoplasty are performed together.
We recommend that you consult fully with a plastic surgery specialist regarding this part, and proceed after discussing only to the extent that can resolve your complex, without necessarily having to undergo unwanted surgeries.
