For revision rhinoplasty, what cases do patients usually come in for consultation?


✅ Dissatisfaction with the shape
✅ Drooping nasal tip
✅ Bulbous nose
✅ Not the desired line
Patients planning their first surgery often worry the most about problems
that can be caused by silicone implants, such as inflammation or contracture.
However, during actual consultations, it seems that a significant number of those seeking revision surgery do so because of the shape.
Among shape-related issues, complaining of a drooping nasal tip and the resulting bulbous appearance seems to be the most common,
and cases where there is a significant difference from the desired design or a deviated nose are also frequently seen.
There are also cases where patients come in due to advanced contracture to resolve shape issues caused by the nose turning up or deviating,
but relatively, that proportion seems to be less than 10 percent.
Why does contracture occur?


You can think of contracture as a symptom of chronic infection.
When silicone enters the body, our body forms a capsule to surround the silicone.
If microscopic inflammations repeat in this capsule, the capsule thickens and shrinks, leading to contracture.
Symptoms of contracture


✅ The capsule hardens and the nose feels stiff
✅ A feeling of shape deformation as it shrinks
Early symptoms are non-specific, making them difficult to notice.
There may be symptoms such as the skin hardening or a smell inside the nose,
but these could be normal scar reactions or
symptoms that appear as the airway inside the nose changes, so it is not easy to judge.
As contracture progresses over time, the capsule shrinks, and the pulling force causes the nose to turn up and deviate.
Also, when you pull on the nose, it does not feel like it stretches smoothly,
but rather feels stiff and hardened.
Since this phenomenon does not happen overnight but progresses slowly over time,
it can be said that a judgment can only be made by observing the progress serially.
Is a red nasal tip also a sign of inflammation?


✅ Discoloration can occur temporarily depending on the environment
Generally, this often happens because the skin does not stretch enough over the elevated nose, affecting blood circulation.
It improves over time, but if your skin is very thin or the nose was raised significantly,
or if the weather is cold, the symptoms may linger a bit.
If there are no other symptoms like swelling, pus, or pain besides the redness of the nose,
you probably don't need to worry too much, but to be accurate, it would be best to visit the clinic where you had the surgery
and get an examination.
Are there any specific prevention methods to avoid inflammation?


✅ Abstain from drinking and smoking for at least a month
✅ Keep the inside of the nose clean
There is actually not much that patients can do to prevent it on their own.
Alcohol lowers immunity and smoking constricts blood vessels, preventing inflammation from healing,
so it is best to strictly abstain from drinking and smoking for at least a month,
and we explain that it is better to avoid excessive drinking or smoking even as time passes.
In the early stages after surgery, a lot of nasal discharge or foreign substances are produced in the surgical area, so keeping the inside of the nose clean is also important.
Is it enough to take antibiotics in the early stages of inflammation?



It depends on the severity of the inflammation.
Basically, inflammation does not heal well while a foreign substance like silicone is present.
Therefore, when the inflammation is not clear and there are only suspicious symptoms like skin redness,
we tend to prescribe antibiotics as a preventive measure.
If the inflammation is definite and there are symptoms like swelling or pus, it should be considered difficult to treat with antibiotics alone,
and you can expect the inflammation to improve only when the silicone is removed through surgery.
Is there a golden time for inflammation as well?


✨ If inflammation occurs, it is obvious, but it is best to visit the clinic immediately for a consultation.
Depending on the condition, treatments such as prescribing antibiotics or removing the silicone through surgery may be necessary.
If inflammation is left untreated, even the autologous cartilage grafted inside can melt
and be absorbed, so it is best to visit as quickly as possible.

Fortunately, I have never had a case where inflammation occurred immediately after surgery.
About 5 years ago, there was a patient who was about 6 months post-op and had no other symptoms,
but the glabella area was protruding, so I went in to replace the silicone.
I saw messy tissues around the silicone, so I removed the silicone and washed the area once.
Fortunately, that patient had used a thin silicone and wanted a natural shape, and the shape after removal was not bad, so I woke the patient up, showed them the shape, and then removed it, finishing the procedure well.
Have you ever provided a revision (A/S) due to shape dissatisfaction?



There have been almost no cases where the originally erected nasal tip drooped over time.
Sometimes the shape is different from what was desired as the swelling goes down.
Even if the shape looks fine to me, the patient had the surgery to be satisfied with it anyway,
so unless it is an impossible part, I usually try to reshape it according to the patient's request.
The strut itself is usually sturdy, so if we correct it by adding ear cartilage or
repositioning and padding it, the patients' satisfaction seems to be high.
