In today's video, I will talk about the causes of sausage eyes and how to succeed in eye revision surgery without failure.
Is there a way to tell if it's sausage eyes or just swelling after surgery?


First of all, it is normal to have residual swelling in the eyelids for up to 6 months after surgery.
It is difficult to perform revision surgery before that, so this is a period where you must wait and see.
Even after that, the swelling will continue to subside little by little, though this varies from person to person.
However, if a year has passed since the surgery, no one would say it is still just swelling.
If the chubby appearance persists for more than 6 months or over a year,
you can assume it is no longer surgical swelling.
What causes sausage eyes?




From the initial surgical design, the height of the fold must be determined according to the condition of your eyelids.
If the surgery is performed with a line that is too high, it can end up looking like a sausage line.
In this case, many people ask if removing fat would fix the issue.
However, the lower part of the double eyelid needs a certain amount of appropriate volume to look natural.
Sometimes, too much soft tissue is removed from the skin right above the eyelashes, leaving it pressed flat.
Not only does this look unnatural and unappealing, but it can also cause the eyelashes to flip upward,
and many patients report discomfort or an inability to close their eyes completely.
When is eye revision surgery possible?



Generally, simple corrections like fixing asymmetry or sagging skin can be done 6 months after the initial surgery.
For revision surgeries that involve redefining the line itself or redoing ptosis correction, it is better to wait longer.
However, since there is also the issue of how long one can wait,
we usually recommend waiting one year after the first surgery for cases requiring such fundamental changes.
Please briefly explain the surgical method for lowering the line (two-line incision).



The two-line incision is an excellent surgical method for lowering a thick double eyelid to a thinner one in patients who lack excess eyelid skin.
However, it is a last resort chosen when no other options are viable,
so it is best to avoid it if there are other possible alternatives.
An incision is made below the existing double eyelid line, leaving the original line as is.
First, the adhesions from the previous surgery are released, and a new, lower double eyelid is created.
Then, fat or muscle tissue is filled in right beneath the original high double eyelid line
to prevent that line from folding, followed by suturing the skin.
Does the two-line incision narrow the vertical width of the eyelid?


Since almost no skin is excised and the width of the double eyelid becomes smaller, the space between the eye and the eyebrow will actually appear wider.
Does the two-line incision leave a scar?



Even if a scar forms on the newly created lower double eyelid, the skin folds and rolls inward along that line,
so the scar is not noticeable. However, the skin scar from the previous high line remains as is
and does not fold inward, making it more visible than before.
What makes a sausage eye revision case difficult?


The more surgeries you have had in the past, the more difficult the surgery becomes. This is because there is a lack of tissue such as skin, muscle, and fat,
leaving no materials to work with.
Therefore, in such cases, we sometimes perform a fat graft on the upper eyelid at the same time.

As mentioned earlier, sausage eyes can only be properly assessed at least 6 months after surgery,
