Today, I would like to review a case of natural adhesion and epicanthoplasty.

Pre-operative Analysis
[Before Surgery]

A 20-year-old female patient visited our clinic with her mother.

She had monolids and wanted to create double eyelids,

but she was very hesitant about the incisional method or ptosis correction due to hearing a lot about their potential side effects and risks.

Her mother asked if it was possible to create double eyelids

without using the incisional method or performing ptosis correction, which involves operating on the eye-opening muscles.

The horizontal width of the patient's eyes was long, but the vertical height appeared narrow.

Although her skin was thin, she didn't even have hidden double eyelids, so when she opened her eyes,

a large portion of her eyes was covered by her eyelids.

Because of this, she used her eyebrows to open her eyes, and her upper eyelids appeared sunken.

She showed a habit of using her forehead muscles to open her eyes.

She had consulted with a few other clinics,

but all of them diagnosed that she needed incisional double eyelid surgery and ptosis correction.

Diagnosis and Surgery

Just because someone uses their forehead to open their eyes does not necessarily mean they must undergo ptosis correction.

It is important to distinguish whether the forehead is being used because the eye-opening muscles actually lack strength,

or if it is because the eyelids obstruct the vision when opening the eyes.

It is best to get help from a board-certified plastic surgeon to make this distinction.

Upon examination, the function of the patient's levator muscle was within the normal range,

and it was determined that she was using her forehead to open her eyes due to sagging eyelid skin.

Therefore, fortunately, she was in a condition where surgery could be performed using the non-incisional method without ptosis correction, just as she wanted.

Additionally, we decided to perform epicanthoplasty together to make the inner corners of her eyes look a bit more open and refreshing.

This is the appearance after removing the stitches on the 7th day post-op. Although there is still some swelling,

the double eyelid lines on both sides are well-formed symmetrically, and the eyes look larger and more open than before.

Even though ptosis correction was not performed, you can see that the habit of using the forehead has disappeared.

The eyebrows have come down to their normal position, and the sunken upper eyelids also appear improved.

The eye shape has changed to a rounder form. Despite the swelling, she gives off a softer impression.

This is the appearance after all the swelling has subsided. A natural in-line double eyelid has settled into place.

The sleepy-looking eye shape also appears improved. Since she no longer uses her forehead,

the position of her eyebrows has naturally lowered, and the sunken upper eyelids have improved compared to before the surgery.

The epicanthoplasty scars are natural without any depressed areas.

A slight redness remains, but it will fade after 3 to 4 months, and will almost completely disappear by 6 months post-op.
