Today, we will be reviewing an under-eye fat repositioning case.

First, let's analyze the issue through the patient's pre-operative photos.

Before Surgery

The patient had previously received under-eye fillers,

but reported that the sunken tear troughs, bulging, and hollow appearance were not improved.

The amount of under-eye fat does not appear to be significant, and the tear trough depression looks deep.

Therefore, we can expect good results by not removing the fat and performing the repositioning without overdoing it.

Consultation and Surgical Process

The patient mentioned that the slightly white, protruding area seemed to be the filler,

and wanted it removed during the surgery.

Upon my examination, there were indeed areas suspected to be filler,

and while I agreed to remove it, I explained one concern I had.

Since the amount of under-eye fat is not large, if all the filler is removed,

there might be insufficient volume for repositioning, which could require concurrent fat grafting.

Therefore, we decided that during the surgery, we would first remove the filler located deep beneath the muscle,

but leave the shallow filler above the muscle untouched for now. After the surgery,

we would monitor the progress and then dissolve the shallow filler.

The surgery was performed using the standard method.

A small amount of filler was found deep under the muscle and removed,

after which the tear trough area was thoroughly dissected and the fat was repositioned.

In this patient's case, although the fat volume was not large,

the orbital septum enclosing the fat was quite loose and sagging.

Along with the fat repositioning, we tightly reinforced the septum

to delay any recurrence as much as possible.

This step is especially crucial when the amount of fat available for repositioning is insufficient.

Lastly, regarding fat repositioning, if too much fat is repositioned,

the under-eye area can actually become sunken, so it is important to adjust and use an appropriate amount during surgery.

1 to 2 Weeks After Surgery

Usually, bruising and swelling improve significantly within a week,

but in this patient's case, because a rhinoplasty was performed concurrently, a considerable amount remained even at the 2-week mark.

Even so, the tear trough area has been filled, and the bulging under-eye area has become relatively flat,

with the boundaries visibly smoother.

For this patient, slight yellow bruising and swelling were still observed at the 1-month mark.

If the rhinoplasty had not been done, this would typically be what it looks like around days 10 to 14.

3 Months After Surgery

Looking at the 3-month progress photo, you can see that the bruising and swelling have completely subsided, and the area has settled into an appropriate shape.

The area where the filler seemed to have clumped up white, which originally bothered the patient,

still felt slightly voluminous after the surgery, and the patient wanted to dissolve the rest of it.

We concluded the follow-up by administering a filler dissolver targeted at the shallow filler above the muscle.

The final shape achieved after this surgery will last until the tissues sag again due to aging,

which we explain is semi-permanent (around 10 years).
