Today, I am going to review a rhinoplasty case.

Today, I will review an under-eye fat repositioning case of a female in her 40s who was concerned about under-eye hollowness.
Before Surgery

Looking at the patient's under-eyes before surgery, the fat is generally protruding forward, making the under-eye area look bulging.
At the same time, the tear trough ligament area is sunken, making the border distinct,
and as a result, shadows are cast under the eyes, making the dark circles appear even darker.
In this case, not only does the under-eye look bumpy,
but it also easily gives off a tired and older impression than her actual age.
Diagnosis and Surgical Method

In the patient's case, there was not an excessive amount of under-eye fat,
but there was enough fat remaining to fill the sunken areas.
In such cases, if the fat is blindly removed, the under-eyes may become even more sunken or
it can result in an older-looking impression, so caution is required.
Therefore, unnecessary fat removal was not performed. Instead, after delicately releasing the tear trough ligament,
we proceeded with an under-eye fat repositioning surgery, relocating the existing fat to the sunken areas.
Because the remaining fat is utilized to supplement the volume,
you can expect a natural and stable result without the need for a separate fat graft.
After Surgery

Looking at the appearance after surgery, the area that looked sunken due to the tear trough has been sufficiently elevated,
and you can see that the entire under-eye area has become much smoother and flatter.
The protruding fat has been organized, and the sunken areas have been naturally filled,
noticeably improving the borders and shadows under the eyes.
As a result, the eyes look much brighter and more lively,
bringing a change that makes the impression softer and younger-looking.
This is a case where a natural under-eye improvement was achieved, not overdone and blending well with the original face.
