Today, I would like to review a case of under-eye fat repositioning and under-eye fat grafting.

Before Surgery

This patient had generally low aegyo-sal (under-eye roll) volume, and

their under-eye area looked sunken overall, with the hollowness around the tear trough ligament being particularly prominent.

Rather than just a localized hollowness in one specific area,

✔️The overall volume under the eyes was lacking

✔️The skin was thin, making the hollowness appear even more pronounced

✔️As a result, this was a case where the patient had a strong tired and aged appearance.

In such cases, to bring out a natural three-dimensional effect,

I determined that this was a type requiring additional volume reinforcement.

Diagnosis and Surgical Method

Since the tear trough ligament was deeply sunken, the area was first reinforced with under-eye fat, but

because the patient's own fat volume was insufficient,

the surgery was performed by additionally harvesting fat from the thighs and combining it with under-eye fat grafting.

→ As the core area with the most severe hollowness, volume was precisely filled to create a smooth, flat improvement.

→ Added natural volume without overdoing it to create a three-dimensional effect.

→ Designed focusing on a smooth transition so it doesn't look artificially puffy.

Rather than simply filling the sunken areas,

the goal was to balance the overall volume under the eyes to improve the appearance into a natural and vibrant look.

1 Week Post-Op

Immediately after under-eye fat grafting, because the fat has not yet engrafted,

a slightly larger amount of fat is injected than the volume that will actually be maintained, and this is accompanied by swelling,

which can give a slightly puffy impression.

​

This is a normal recovery process,

and it will naturally settle down along with the swelling over time.

3 Weeks Post-Op

As the swelling gradually subsides,

the previously deeply sunken tear trough ligament area becomes noticeably flatter,

and as volume fills the entire under-eye area, you can see the face transforming to look much more three-dimensional and vibrant.
