Today, I would like to review an eye surgery case.

The under-eye area plays a very large role in determining your overall facial impression.

If the fat bulges out or, conversely, looks sunken, it can give off a tired and aged appearance.

In this post, through a real case of a patient who underwent both under-eye fat repositioning and under-eye fat removal,

I will explain in detail how we diagnose and correct under-eye asymmetry.

Before Surgery

In this patient's case, the tear trough ligament area was sunken,

creating a stark contrast between the bulging fat and the hollowed areas.

This caused a double-lined shadow under the eyes,

For this patient, simply removing the fat

or repositioning all the fat would have made it difficult to achieve satisfactory results.

✅ Deep shadows caused by hollowness in the tear trough ligament area

​

✅ A typical under-eye aging pattern where both bulging and hollowness coexist

Therefore, we established the following customized surgical plan.

👉 Right Under-Eye

​

Because the fat was protruding relatively more

→ Partial removal of only the excessive fat

​

​

👉 Both Sides (Common)

​

Without removing the remaining fat

→ Performed under-eye fat repositioning

​

By releasing the tear trough ligament area

→ Naturally filled in the hollowed areas

​

By doing this,

we aimed to reduce the bulging areas and fill the hollowed areas

to create a smooth overall under-eye contour.

After Surgery

Looking at the photos taken one month post-op, you can see the following changes.

The under-eye area has been smoothed out to be generally flat and soft.

The previously visible

👉 Double-lined shadow → Reduced to a single line

​

Left-right asymmetry was improved,

👉 Forming a natural and balanced under-eye line

​

From a hollow and tired-looking impression

👉 Transformed into a much brighter and more vibrant impression

​

In particular, not unconditionally removing the under-eye fat

but filling the hollowed areas through repositioning

played a crucial role in creating natural results.

Under-eye surgery is not simply a "fat removal surgery," but
