A female patient in her 40s visited our clinic, complaining that her left outer eye corner looked stuffy, as if the upper and lower skin were stuck together.

She had undergone double eyelid surgery and lateral canthoplasty 10 years ago,

and after receiving lower canthoplasty a year ago, she underwent lower canthoplasty reconstruction just 7 months later.

Before Surgery

However, as the swelling gradually subsided after the lower canthoplasty reconstruction, she felt as though the upper and lower eyelid skin had fused, making it seem like her eyes weren't fully open,

describing the sensation as if they were glued together.

She also noted an increase in fine lines around her eyes, and when she tried to open her eyes wide,

she felt a pulling sensation in the surgical area.

Diagnosis and Surgery

This phenomenon can occur if the eyelid skin at the outer corner is subjected to significant tension due to surgery,

or if there is a shortage of skin.

When the upper and lower skin at the outer corner fuse together, creating a vertical skin web,

the outer corner looks stuffy, and the field of vision can even be obstructed.

In this case, the vertical scar must be released, and the scar length needs to be extended.

Additionally, if necessary, filling in the lacking skin at the outer corner will yield better surgical results.

Such results can be achieved through Z-plasty and local flap surgery on the scarred area.

2 Months Post-Surgery

This is how it looks after all the swelling has subsided.

The stitch marks visible in the early post-operative stages have also smoothed out nicely.

The previously stuffy-looking outer corner, where the upper and lower parts were fused in a rounded shape,

has now become normally sharp and refreshingly open.

The patient also mentioned that it is now easier to open her eyes and that her eyelid wrinkles have decreased.

Ultimately, the patient was highly satisfied with the natural-looking eye shape and the clean surgical site.
