As our bodies reach our late 20s to late 30s, we begin to experience signs of aging.
Among these, the eyes are where aging appears the fastest! One of those signs is sunken eyelids.
The causes of sunken eyelids include the natural aging process, eye structure, and family history.
Nowadays, there are also cases where sunken eyelids occur despite a young age.
Today, because of the sunken eyelid phenomenon,
we will take a look at actual photos of a patient who underwent surgery at our clinic.
Upper Eyelid Fat Grafting

A female patient in her 20s who had incisional double eyelid surgery 10 years ago visited us because her upper eyelids looked sunken,
her double eyelid lines were loose, and her eyes appeared tired and lacking energy, which she wanted to resolve.
Pre-Op Analysis


Patient's Pre-Op Condition
It appears that fat removal was performed together with her past incisional double eyelid surgery.
Both upper eyelids look sunken, and the most sunken areas have even developed folds similar to multiple creases.
Q: Shouldn't I get double eyelid surgery, ptosis correction, and upper eyelid fat grafting all at the same time?
The patient inquired whether she needed to undergo double eyelid surgery, ptosis correction, and upper eyelid fat grafting simultaneously.
However, her current double eyelid size was neither too large nor too small, but an appropriate size that suited her eyes,
and the condition of the scar was neither depressed nor widened.
There was no asymmetry, the lines maintained smooth curves well,
and they connected well to the end, so it was determined that changing the double eyelid line was not necessary.
Also, although the patient complained of looking sleepy and lacking energy,
an evaluation of the function of the levator palpebrae superioris muscle (the muscle that opens the eyes) showed that it was maintaining normal function.
She was simply experiencing discomfort because the sunken parts of her upper eyelids were rolling inward every time she opened her eyes.
Therefore, we aimed to maintain the double eyelid line as it was while improving only the sunken areas of the upper eyelids to eliminate the discomfort when opening her eyes,
and to create an eyelid shape with no multiple creases and an appropriate amount of volume.
A: You only need to proceed with upper eyelid fat grafting.
Consequently, we decided to proceed with only the upper eyelid fat grafting surgery.
Diagnosis and Surgery

This patient has relatively thin eyelid skin and a slightly deep-set eye structure
where the brow bone protrudes further forward than the eyeball.
Being deep-set and lacking upper eyelid fat, her profile looks even more sunken,
creating multiple creases and a tired impression.
Under sleep anesthesia, her own autologous fat was harvested and separated into micro-fat suitable for grafting into the upper eyelids.
Afterward, local anesthesia was administered to the eyelid area, and 1.2cc of micro-fat was grafted into each side to fill the sunken areas.
✔ Since the double eyelid line was not operated on separately, there is no swelling on the line, and swelling is only present in the area where the upper eyelid fat was grafted.
Post-Op Progress


These are photos of the progress 1 month after surgery.
The swelling has almost completely subsided.
The double eyelids maintain the shape of the previous surgery, and only the volume of the upper eyelids has been filled.
The patient says it is much more comfortable to open her eyes than before.
She feels that even without ptosis correction surgery, simply filling the volume of the sunken upper eyelids has made it easier to open her eyes.
The multiple creases have disappeared, and a naturally convex eyelid shape has been well-formed even when viewed from the side.
Because the surgery was performed by grafting micro-fat into the exact locations of the upper eyelids, the grafted fat cannot be felt,
nor does it bulge out when closing the eyes, maintaining a natural shape.


Initially, this patient thought she would need to undergo three surgeries: incisional double eyelid revision, ptosis correction, and upper eyelid fat grafting,
but she was able to achieve satisfactory results with just one surgery—fat grafting.
Undergoing multiple surgeries does not necessarily guarantee better results.
Especially in revision surgeries, it is best to receive only the surgeries that are absolutely necessary for you.
This is because if multiple surgeries are performed simultaneously, the potential post-operative complications can also increase in complexity.
