Today, I would like to review a natural adhesion (dermal adhesion) case.

​

Pre-operative Analysis

The patient originally had a natural double eyelid, but the line would occasionally become loose.

Also, the skin on the lower part was sagging, giving an undefined appearance, so she wanted to create a more defined line.

​

Diagnosis and Surgical PLAN

This patient has relatively thin skin and good levator muscle strength.

However, the size of the right orbit is small, making the distance between the eyebrow and the eye short, and the upper skin relatively

covers the eye more, so we decided to make the right side slightly larger than the left.

We decided to raise the right side by about 0.5mm to offset the difference in skin coverage and achieve symmetry.

In addition, we decided to slightly raise the front line to create a rounded design.

If we used an incisional method to fix it tightly to prevent loosening, it could cause a sausage eyelid effect.

Therefore, we proceeded with natural adhesion (dermal adhesion) to fix the line almost exactly as it currently is.

​

7 Days Post-op

There is some swelling, but the line is defined and symmetrical as intended.

By setting the right side slightly higher, the left and right symmetry matches well.

While the original line had a slightly lower front part that gave a sharp, angled impression, the line has now settled into a rounded shape.

Due to the swelling, the sagging of the line or the lower double folds are less visible, but rather than this state being maintained,

it is expected that as time passes, the line will naturally loosen a bit and the lower double folds will become visible.

11 Days Post-op

Compared to day 7, the swelling has gone down significantly, and as expected, the lower double folds are slightly visible.

​

1 Month Post-op

With more swelling subsiding compared to day 11, the double eyelid has become slightly thinner and more natural.

​

3 Months Post-op

The swelling has gone down a bit more compared to the 1-month mark, but since there wasn't much swelling to begin with,

the line is well-maintained without any major changes.

The line looks similar from pre-op to 3 months post-op, but it is more defined than before,

and the asymmetry where the right line looked smaller due to skin coverage has been corrected.

Also, by raising the front line slightly compared to before, the impression has changed from a steeply dropping line

to a more rounded, smooth curve.
