Double eyelid surgery is divided into two types: non-incisional and incisional.

Non-incisional double eyelid surgery is a method of creating a line without cutting the skin,

while incisional double eyelid surgery is a method of connecting the skin to the levator palpebrae superioris muscle through a skin incision.

Cases that require incisional double eyelid surgery include

when the eyelids are thick and have a lot of fat, when the degree of eyelid drooping is severe,

or when the patient is dissatisfied with their previous double eyelids and wants revision surgery.

Today, we will look at a real patient case who underwent incisional double eyelid surgery along with a 45-degree epicanthoplasty and upper canthoplasty, accompanied by their photos.

Before Surgery

Before surgery, the patient had no double eyelids and slight multiple folds.

Also, due to the Mongolian folds at the inner corners, the eyes looked stuffy and gave off a sleepy impression.

Diagnosis and Surgical Plan

Incisional Double Eyelid + 45-Degree Epicanthoplasty & Upper Canthoplasty

Since the patient had multiple folds that could easily loosen, the incisional method was chosen for the surgery.

We decided to release the anterior Mongolian folds to open up the inner corners, creating a larger and more refreshing look than before.

Although the eyes looked sleepy, this was caused by the drooping eyelids covering the pupils.

Since the levator muscle strength (eye-opening power) was adequate, we decided not to proceed with ptosis correction.

Post-Op ~ 1 Month

1 Week Post-Op

There is slight bruising and swelling.

Since the skin is not very thick, there is relatively less swelling compared to other patients.

The canthoplasty scars are not severe, but we explained the possibility that they might peak and become most noticeable around one to one and a half months post-op.

2 Weeks Post-Op

About half of the swelling has subsided, and the eyes open more clearly without any interference in the eye-opening strength.

The canthoplasty scars have not raised much and are in good condition. The bruising remains only in the center, with the rest almost completely faded.

1 Month Post-Op

1 Year Post-Op

Since there wasn't much swelling from the beginning, there wasn't a significant reduction in swelling over time.

The skin is thin, and the fixation is natural but well-maintained without loosening, so there is almost no feeling of the line becoming faint or unraveling.

The height of the fold has been maintained consistently since the second week.

As the epicanthoplasty scars fully healed and softened, the shape of the inner corners settled naturally and looks refreshing.

Today, we looked at a case of incisional double eyelid surgery with a 45-degree epicanthoplasty and upper canthoplasty.
