Male Eye Surgery: Incisional Double Eyelid + Upper Eyelid Fat Removal

A male in his 20s visited us for an eye surgery consultation.

He wanted to change his appearance because his eyes were small and his eyelids were thick, making them look heavy.

He had visited another clinic 3 years ago for this purpose, but worried that double eyelid surgery on a man would look burdensome and unnatural,

so he received a sub-brow lift instead.

However, even after the sub-brow lift, the thickness of his eyelids remained, and his impression barely changed.

Pre-operative Analysis

Patient's Condition Before Surgery

The horizontal width of the eyes is long, but the vertical height is narrow, which can give a fierce, sharp-eyed impression.

The eyelids are thick and have a lot of fat, making the eyes look small and stuffy rather than refreshing.

The sizes of both eyes are also different; the right eye (the left eye in the photo) is smaller, and its eyelid skin is thicker.

What kind of eyes did the patient want before surgery?

The patient stated that he wanted his eye shape to change from his current fierce impression to a softer one.

Also, rather than a completely hidden double eyelid, he wanted a male double eyelid where the lines at both ends are slightly visible when looking straight ahead.

Among celebrities, he preferred the eye shape style of singer Rowoon.

Diagnosis and Surgery

This patient has thick eyelid skin and a skeletal structure where the brow bone protrudes forward.

The upper eyelids look bulging due to a lot of fat.

There is no hidden double eyelid at all, and when the eyes are open, the pupils are covered by the eyelid skin, showing only half of them.

This part might look like ptosis, but the function of the levator palpebrae superioris muscle (the muscle that opens the eyes) was normal,

so it was diagnosed that there was no ptosis.

Therefore, we planned to create a double eyelid of an appropriate size through incisional double eyelid surgery,

and remove the necessary amount of skin and fat from the upper eyelids.

Since there was no ptosis, ptosis correction was not performed,

and because the eyes were horizontally long, we determined that additional surgeries like epicanthoplasty or lateral canthoplasty were not necessary.

※As a result, incisional double eyelid surgery and upper eyelid fat removal were performed.

Post-operative Progress

7 Days Post-surgery

This is the appearance after stitch removal on the 7th day post-surgery. Swelling still remains, but

both double eyelid lines are symmetrically well-formed, and the eyes look larger and more refreshing than before.

The eye shape has changed to a rounder form.

There is swelling, but the impression looks softer.

The bulging fat on the upper eyelids has disappeared, and the double eyelid lines form smooth curves

that harmonize naturally with the eye shape.

2 Months Post-surgery

This is the appearance with all the swelling gone.

It is a natural in-line double eyelid, where the lines at both ends are slightly visible just as the patient wanted.

The eye shape has also become larger and rounder, and the thick fat on the upper eyelids has disappeared, resulting in light and soft-looking eyes.

The incision scar still looks red when looking down, but

it appears as a smooth single line without any sunken or bumpy areas.

The redness of the scar will fade after 2 months, and it will blend with the skin color 6 months post-surgery.

Ultimately, the patient was very satisfied with the natural-looking double eyelids and the clean surgical marks.
