The nose is the core of facial balance, determining the overall impression and atmosphere,

and plays an important role in deciding an individual's image.

For the nose, a line that forms a natural curve from any angle is important.

A flowing curve from the nasal bridge to the nasal tip, philtrum, and chin tip

is ideal for creating beautiful three-dimensional volume.

It is best if the first surgery is performed with a line that suits each individual well,

but if that is not the case, revision surgery may be considered.

Today, as a revision rhinoplasty case,

we will take a look at photos of an actual patient who had surgery at Uber Plastic Surgery.

Revision Rhinoplasty (Bulbous Nose + Columella Correction)

This patient previously had surgery using ear cartilage and silicone.

​

Pre-operative Analysis

Patient's pre-operative condition

✔ Bulbous nose

✔ Upturned nasal tip

✔ Deviated nasal bridge

✔ Nostril asymmetry

Before Surgery

This patient had previous rhinoplasty, but the bulbous nose was under-corrected,

and the nasal tip felt upturned.

Also, the patient mentioned that the nose looked deviated.

​

Due to the deviated appearance, when viewed from the front, the nasal tip was shifted to the left,

and when viewed from below, the columella was tilted and raised to the left,

resulting in asymmetrical nostrils.

​

The retracted columella made the bulbous nose look more severe, and when viewed from the side, it made the mouth look relatively

protruding, reducing the three-dimensional volume of the midface.

​

What kind of nose did the patient want before surgery?

The patient prioritized bulbous nose correction.

They wanted the blunt nasal tip and bulbous nose to be naturally transformed into a straight-yet-slightly-curved (Jikban) line,

and desired improvement for the drooping nasal tip.

​

Diagnosis and Surgery

Through surgery, the retracted columella, deviated nose, and nostrils were corrected,

the upturned nasal tip was lowered, and the columella was brought out to make the mouth look less protruding.

The design aimed to bring the nose forward, giving the midface a more three-dimensional appearance.

​

Although the septal cartilage was not used previously, the amount was insufficient, so donated rib cartilage was used together,

and ear cartilage was used for the nasal tip. Silicone of a similar height to the previous one was used for correction.

Post-operative Progress

Looking at the post-operative photos, from the front and bottom views,

the deviated nose and columella are aligned straight, and the nostril asymmetry is greatly corrected.

From the side profile, the upturned feeling of the nose is improved,

showing a well-extended straight-yet-slightly-curved (Jikban) line,

and the extended columella makes the mouth look less protruding, improving the nose three-dimensionally.
