Today, I'm going to review a case of eyes that became smaller and looked stuffy after lower blepharoplasty.

A 62-year-old male patient visited us complaining that both outer eye corners looked blocked after undergoing lower blepharoplasty 5 years ago.

Although he had symptoms on both sides, the left side was particularly severe,

and at a glance, a skin web caused by scarring was covering his eye.

Pre-operative Condition

This patient had lower blepharoplasty 5 years ago and also underwent upper blepharoplasty 15 months ago.

It appears that a certain amount of skin excision was performed on both the upper and lower eyelids during the surgeries,

but fortunately, his eyes were closing well.

The patient also mentioned that although he experienced some dry eye symptoms and discomfort in the early stages after surgery,

they have currently disappeared.

However, because the skin at both outer eye corners was covering the eyes, he felt as if his eyes were not fully open even when he tried to open them completely,

and he felt his field of vision narrowed when looking to the right and left.

What the Patient Wants

He wanted to eliminate the stuffy feeling and narrowed vision at both outer eye corners

and be able to open his eyes clearly and comfortably as before.

Diagnosis and Surgery

During lower blepharoplasty, if the skin incision design is directed too high upward at the outer corner of the eye,

or if a large amount of skin is excised in that area, this phenomenon can occur.

In this case, when suturing the skin at the outer corner of the eye, it receives significant tension, leaving a vertical scar.

The vertical scar prevents the eye from opening widely, causing a feeling of stuffiness when opening the eyes.

If it becomes more severe, a skin web forms, which can obstruct the field of vision when looking to the far right or left.

In this situation, the vertical scar must be released, and the scar length needs to be extended.

Also, if necessary, filling in the lacking skin at the outer corner of the eye will yield good surgical results.

Such results can be achieved through Z-plasty on the scar area and a local flap procedure.

8 Days Post-Op

This is the appearance after stitch removal on the 8th day post-op.

Since the entire eyelid was not operated on, there is minimal swelling.

Even immediately after stitch removal, you can see that the pre-existing skin web has disappeared, and it feels much more comfortable to open the eyes.

The patient was satisfied, stating that the visual obstruction had disappeared and he felt much more comfortable.

Although slight stitch marks remain, we can expect the scarred area to soften further over time.

2 Months Post-Op

This is the appearance after all the swelling has subsided.

The stitch marks that were visible in the early post-op stages have also smoothed out nicely.

The previously small and stuffy-looking outer eye corners have transformed into a clear and open appearance.

The patient also mentioned that opening his eyes became comfortable, and the obstruction at both edges of his vision had disappeared.

As a result, the patient was highly satisfied with the natural-looking eye shape and the clean surgical marks.
