'Long Nose Rhinoplasty'.

A long nose is a challenging case that even experienced surgeons find difficult due to its high surgical complexity!

Today, we will explain what a long nose is, why a nose appears long,

and why long nose rhinoplasty is so difficult!

Long noses can be broadly divided into three types:

Depressed nose, arrow nose, and dragon nose.

The first type, the depressed nose, is often influenced by a long face.

If the midface is long, it lacks anterior-posterior projection,

and it is common for the nose to look slightly depressed with an upturned tip.

The primary solution is to lower the upturned nasal tip,

but if you are concerned about the face looking too long, elevating the depressed nasal bridge is also an option.

If the glabella (between the eyebrows) is sunken,

raising it too much can create an unnatural 'Avatar' look.

Therefore, instead of a curved line that might reduce dimensionality,

it is best to adjust the height of the nasal tip and bridge just slightly

and compensate for the flaws with a straight-curve (Jikban) line.

The second type, the arrow nose, can be divided into two cases:

1. Those with a short midface but a drooping nasal tip,

2. Those with a long midface and a drooping nasal tip.

In the first case, if the nose is short, there is a limit when performing tip elevation surgery

because the overall length of the nose can become even shorter.

In such cases, lowering the nasolabial angle can prevent the overall length of the nose

from shortening while keeping the tip from looking droopy.

However, for those with a short philtrum, there is insufficient space for the nasolabial angle,

making arrow nose correction potentially difficult.

The second case is slightly more complicated than the first.

If the nasal tip is raised, the nose shortens but the tip becomes upturned,

and if the nasolabial angle is lowered, the overall length of the nose cannot be reduced much.

Taking celebrity Lee Kwang-soo as an example,

because his midface itself is long,

it is difficult to reduce the overall length of the nose.

The arrow nose is also the type with the highest recurrence rate after surgery.

This is because, naturally as humans,

a strong downward force acts on the nasal tip when we smile.

To prevent this, even if it is slightly uncomfortable,

we have no choice but to build a stronger strut for support.

The last type, the dragon nose, is characterized by a hanging columella

and flared alar wings, making the nostrils very visible.

The solution is to raise the tip of the flared nose

and lift the hanging columella together.

If the nostrils are widely flared, alar reduction can be performed concurrently,

but because the alar wings shrink while slightly moving upwards,

there is a possibility it might look like a dragon nose again,

so you must proceed with the surgery taking a slight flare into consideration.
