Functional rhinoplasty, as the name suggests, refers to nose surgery performed for functional purposes rather than aesthetic ones.

Today, we will find out how it differs from the cosmetic rhinoplasty we usually perform.

​

And is it better to do functional and cosmetic rhinoplasty together? Or is it better to do them separately?

Today, I would like to answer these questions!

What is functional rhinoplasty?

Unlike its outward appearance, the nose has a very complex anatomical structure.

Are there people whose noses get stuffed up easily?

These individuals generally have a condition called a deviated septum, where the septum located in the center of the nose is bent,

or there is a narrowest part in the center of the internal cross-section of the nose called the nasal valve.

If this area collapses or narrows, breathing becomes uncomfortable.

If you also have rhinitis, you usually end up breathing through your mouth rather than your nose, which causes many problems.

On a smaller scale, breathing through your mouth can cause dry mouth and bad breath,

and if you continuously breathe through your mouth, it can even change the shape of your face.

When there is a functional problem with the nose like this, the method to fundamentally solve this issue is functional rhinoplasty.

Can I do functional rhinoplasty and cosmetic rhinoplasty together?

​

Since you are probably familiar with cosmetic rhinoplasty, I will skip the explanation,

but we receive this question very often.

​

Is it better to do cosmetic rhinoplasty while doing functional rhinoplasty? Or is it better to do them separately?

➰Actually, there is no right answer.➰

To give an easy example with double eyelid surgery, even if it's the same surgery,

double eyelid surgery is a procedure that simply creates a crease in a monolid to form a line.

There are many factors to consider, such as the overall facial image, eye shape, eyelid thickness, fat amount, and line preference, to determine if the double eyelid created after surgery

will harmonize well with other parts of the original face.

So, to the question of whether you can do functional and cosmetic rhinoplasty together,

if it's good to do them together, or if they must be done together, then it's better to do them together.

But it doesn't mean you absolutely have to do them together even when there's no need to!

If they must be done together, we need to accurately determine whether there is a functional problem or not.

This judgment cannot be made just by looking with the eyes. To explain using our clinic as an example,

if a deviated septum is found and nasal valve collapse is observed through CT scans and endoscopy,

we proceed with the surgery after making a definitive diagnosis of rhinitis.

When comprehensively considering radiological findings, physical examination, and the patient's nasal congestion symptoms,

if we are certain that functional rhinoplasty is necessary for this patient and that adding some cosmetic improvements would be beneficial,

we recommend doing the surgeries together.

Surgical methods for functional rhinoplasty

First, if the septal cartilage is bent in a C or S shape, and only cosmetic rhinoplasty is performed in this state,

there are cases where the prognosis is not good.

✅ The nasal bridge shifts

✅ The nasal tip droops

✅ Worsening nasal congestion

Therefore, the bent septal cartilage and part of the bone are removed, and through surgeries such as reinforcement grafts or spreader grafts,

the bent septum is straightened.

In addition, if there is rhinitis, the narrowed nasal cavity is widened through osteotomy of the hypertrophied bone and Coblation submucosal reduction.

Then, through nasal valve collapse surgery, which inserts septal cartilage between the narrowed nasal valves,

we create a state where it is easy to breathe, and at the same time, we create the cosmetic line and nose shape that the patient desires.
