When she first started performing eye surgeries,

our doctor used to memorize a few reasons why non-incisional (suture) double eyelids easily come undone to explain them to patients.

✔️ When the eyelids are thick

✔️ When the epicanthal fold is severe

✔️ When frequently rubbing eyes due to allergies

✔️ When having protruding eyes

✔️ When having very low inner double eyelids

✔️ When having ptosis

However, one day, a patient in her 40s with ptosis and atopic dermatitis on her eyelids

insisted on getting only the non-incisional double eyelid surgery. The doctor performed the surgery,

and when checking the progress a year later, she was surprised to see that the double eyelid hadn't come undone at all

and had settled perfectly as if it were her natural line.

Natural Adhesion Surgery = Non-incisional Surgery

Natural adhesion and non-incisional surgery actually mean the same thing.

Originally, double eyelid surgery can be broadly divided into non-incisional and incisional methods.

If the surgery involves cutting the skin, it is the incisional method,

and if it involves making only small holes and tying the tissue without cutting the skin, it is the non-incisional method.

Since the tying technique for the non-incisional method varies slightly from doctor to doctor, they often give it their own unique names.

That is why various names have emerged, such as natural adhesion, complex non-incisional method, double, triple, dual, etc.

Among them, the name 'natural adhesion' seems to have been the most preferred and widely known,

so it is now used almost synonymously with the non-incisional method.

The Secret to Unbreakable Natural Adhesion

What should be done to prevent natural adhesion double eyelids from coming undone?

First!! The design must be done well!

The eyelids have both thin and thick skin.

No matter how thick a person's eyelids are, there is always a somewhat thin area.

The double eyelid must be created on the thin area so it does not come undone.

However, since a line that is too small can also easily unravel, we must find an appropriately high position within the thin area.

(This part is not easy.)

A person's eyes are asymmetrical, so we must find the right line for each eye

and design it so that it connects smoothly in a curve from the beginning ➡ middle ➡ end of the line.

To find this, the patient repeats opening and closing their eyes multiple times while sitting in an upright position.

With the non-incisional method, the double eyelid forms exactly as designed.

(There must not be even a 1mm margin of error during the design stage.)

Second!! The thread passing through the eyelid skin must be present in as many areas of the dermal tissue as possible.

Sometimes people ask whether there are 5 or 6 holes for the non-incisional method, but the number of holes is not important.

Also, some people ask if there is one knot or multiple knots, but this part is not important either!

Lastly, tying the thread tightly in double or triple layers does not guarantee that the double eyelid won't come undone!

If tied too tightly, there will be a lot of swelling, and there is a risk of the thread breaking or unraveling.

*If there is much to gain from the incisional method, such as fat removal / ptosis correction / resolving sagging, then the incisional method should be performed.

If the surgical results are expected to be similar between the incisional and non-incisional methods, there is no need to avoid the non-incisional method just out of fear that it might come undone.

Q&A_1 Can a person with thick eyelid skin not get natural adhesion?

No matter how thick a person's eyelids are, when they close their eyes, there is bound to be a somewhat thin area on the eyelid.

If the double eyelid is created on that thin area, it is perfectly possible without any issues.

However, if removing the thick part of the eyelid would look more aesthetically pleasing, we recommend the incisional method.

Q&A_2 How long do scars last after natural adhesion surgery?

Since the advantage of the non-incisional method is the absence of scarring, it must not leave any noticeable scars.

Sometimes I have seen cases where tissue is removed through small holes or the holes are widened under the belief that more adhesion is needed to prevent the non-incisional method from unraveling. However, this does not help prevent the double eyelid from coming undone

and instead only leaves a larger scar.

★The holes for the non-incisional method only need to be large enough for a needle to pass through.

However, the depth of the hole must go deep into the muscle layer and should not be too shallow.

(Because the needle marks might look pinched.)

For the first 2~3 months after non-incisional double eyelid surgery, swelling remains, so pinched marks may be visible along the double eyelid line.

However, this does not remain as a scar, and by the 5th~6th month when the swelling has almost completely subsided,

the line will remain as a single continuous line overall, and the pinched look will no longer be visible.
