Hump nose: Cases that require osteotomy vs. cases that don't?
Osteotomy may only be necessary if the hump is severe.
A nasal hump consists of a bony part and a cartilaginous part.
The cartilage portion is excised, and the bone portion is either shaved down if it is mild, or requires an osteotomy if it is severe.

Also, if a large amount of the upper bone is shaved down, it creates an open roof deformity. In this case, a lateral osteotomy is required to close the roof. Additionally, because the nasal airway may become narrow and cause breathing discomfort, a spreader graft using cartilage is often performed together
to prevent the airway from narrowing.
