Hello~ Your beauty universe!

❤️I am Dr. Shim Jeong-hwan, head of Uber Plastic Surgery :)❤️

Q: What is the reason for the nasal tip showing through after rhinoplasty?

If the skin is thin,

the internal structures are more likely to show through to the outside.

Because rhinoplasty involves building a firm internal structure

and covering it with skin,

for those with thick and firm skin, even if the internal structure is made sharp,

there can be a disadvantage where the exterior still looks relatively bulbous.

Conversely, for those with thin skin, no matter how smoothly the inside is shaped,

there is a high possibility that the contours or angular parts of the cartilage will be visible.

This phenomenon can also occur naturally

even in those who have not had surgery.

However, because unprecedented changes occur after surgery,

it often becomes more of a concern.

Especially if the nasal tip is raised excessively compared to the condition of the skin,

the continuous pressing force

can cause the skin to gradually thin out over time, worsening the visibility.

In the case of revision surgeries, if repeated multiple times,

the skin inevitably becomes thinner,

so the cartilage can show through more easily even with a small amount of force.

Q: The nasal tip shows through more as time passes

Immediately after surgery, the swelling may make the visibility less noticeable, but

usually after 6 months to a year,

the swelling subsides and the condition stabilizes.

After 3, 5, or more years have passed,

as the stimulation applied to the skin gradually increases,

there is a possibility that the cartilage visibility will progressively worsen.

Q: Do such patients absolutely need revision surgery?

Cartilage showing through does not necessarily require revision surgery.

Because it is highly unlikely for the cartilage to pierce through the skin,

you do not need to undergo surgery if it does not bother you significantly.

However, since skin thinning caused by surgery or

naturally thin skin cannot be reversed,

if you wish to maintain the current height,

it can be helpful to undergo a concurrent procedure

to reinforce the skin through dermis grafting.

Conversely, to reduce the tension applied to the skin,

choosing to lower the height slightly from the original may be a more suitable option in some cases.

Q: Then will there be no visibility issues after this?

Depending on the degree of engraftment, a dermis graft may show through again,

and additional grafting may be performed if necessary.

Therefore, it is difficult to say that the visibility will disappear 100% completely.

Q: How should the surgery be done to prevent visibility?

Because septal cartilage, which is widely used in nasal tip surgery, is a firm material,

its texture or contours can be felt relatively easily.

There is also the method of using ear cartilage, but

considering the retention of the nose shape, it is common to use septal cartilage.

In this case, it is important not to raise the nasal tip height excessively.

Before surgery, along with the skin condition,

it is advisable to also consider the movement of the nasal tip.

For those who frequently move their nasal tip when smiling or talking,

the stimulation applied to the cartilage can increase,

so a natural and stable design is more suitable from the beginning rather than an excessive height.

If you are concerned about cartilage visibility, lowering your expectations for height a little

and choosing a natural line, then if necessary,

reinforcing the skin with a dermis graft can help you expect sufficient improvement.

If you want to reduce visibility after rhinoplasty,

letting go of the desire for height a bit to naturally lower the nasal bridge,

and reinforcing the skin by grafting dermis if lacking can sufficiently resolve the issue,

so I hope you consult with a board-certified plastic surgeon to achieve good results.
