Today, I am
going to explain the question,
"Q: What is the nasal septum?"~

We use septal cartilage very often during rhinoplasty.
The nasal septum is,

the area marked with a square is the septal cartilage,
โ

and this area marked in yellow is the septal bone.
โ

In other words, the beginning and end of the arrowed part
make up the nasal septum.
โ
If this length is about 6 to 7 cm,
you can consider it as having an abundant amount.
โ
โ

Q: Can't it be used if the septum is insufficient?
โ
For those who lack sufficient septal cartilage,
surgery must be performed using donated cartilage (donated rib cartilage) instead of septal cartilage.

Or autologous cartilage.
Commonly known as autologous rib cartilage.

Often, it's not that the septum itself is lacking, but during surgery,
because we try to harvest only the septal cartilage excluding the septal bone,
the amount is frequently insufficient.

Q: But why shouldn't the bone be used?
โ
I believe the bone can be used.
โ
However, the problem is,
if you look at the septal bone and cartilage,
they are naturally structured to separate cleanly.

During the process of harvesting the septum,
we want to harvest it like this (the purple circle area),
but if it separates, we might not be able to secure enough length.
โ
The bone is thin but hard.
Because it is thin yet hard and rigid,
its rigidity makes harvesting somewhat difficult.
โ

Next, when extending the septum,
it needs to be connected and fixed with sutures,
but since a needle cannot pass through the bone, fixation is difficult.

For these various reasons,
surgeons tend to avoid using the septal bone and only use the septal cartilage.

If, in order to harvest a lot of cartilage,
only a small portion is left at the front
and too much is harvested, the whole structure would collapse, right?
โ
Also, if harvested safely, the amount might fall short.
โ
However, if the septal bone is used appropriately,
the probability of using donated rib cartilage
and autologous rib cartilage during rhinoplasty
can be significantly reduced.

Q: How can it be used if the septum is small?
โ
Rhinoplasty is a combination of your current nose condition, your desired shape, and the materials used.
You can think of it as a combination of these three factors.
โ
If your expectations for the desired post-operative shape are very high,
but assuming your current nose condition is quite poor,
what needs to happen to achieve that desired shape?

The answer is that the materials used for the nose surgery
must be exceptionally good.
โ
But what if your current nose condition is good,
and you don't want a drastic change for your desired post-operative shape?
โ
Then, the materials actually become less important.
โ
In other words, the patient's current condition, the desired shape, and the rhinoplasty materialsโ
surgical decisions are made by comprehensively considering these three factors,
โ
and when these three combination conditions are well harmonized,
a good result can be achieved after rhinoplasty.
โ
Therefore, I hope you do not think of rhinoplasty in a one-dimensional way.
I wanted to convey this message.

Q: Can't it be used if the septum is deviated?โ

From a straight septum
to various deviated shapes, I have drawn a few examples.

Excluding the straight septum,
which of the remaining septums cannot be used during rhinoplasty?

The answer is the very last one,
the S-shaped deviated septum.

To give an example using paper,
thin papers mostly bend easily into an S-shape.
When comparing this to the nasal septum,
if the septum is bent in an S-shape?

-> It means the septum is extremely thin.
Because of this, you can consider it very difficult to use during surgery.

On the other hand, strong and stiff papers bend at a right angle,
and a septum deviated in this manner can be used during surgery.

Because the folded parts are flat,
we can determine the length of the septum and other details by looking at a CT scan,
and then utilize it during rhinoplasty.

Just because the septum is deviated,
