Today, I would like to review a case of incisional double eyelid surgery and ptosis correction.


Before Surgery

This patient is a male in his early 20s who visited us with sleepy-looking eyes.
He had monolids and actual ptosis due to weak eye-opening muscles,
but he did not use his forehead much as a compensatory action.
The shape of his eyes was horizontally long, and the lacrimal caruncle at the medial canthus was hidden by the epicanthal fold,
but it did not look particularly stuffy or closed off.
First of all, the patient wanted the eyelid line to be as invisible as possible (as he liked his current eye shape),
but wished for his eyes to look a bit more defined, and he inquired if an epicanthoplasty was necessary.
Consultation and Surgical Process

We needed to design the lowest possible line,
but because the front of the line was heavily pressed down in connection with the epicanthal fold,
a non-incisional method would likely result in the line loosening quickly,
and the degree of ptosis correction would likely be insufficient.
To prevent it from loosening while removing a small amount of the overlapping skin at the start of the front line,
we planned an incisional ptosis correction surgery to firmly secure the line.
Although the epicanthal fold was tightly interfering with the front line,
the shape itself already had a horizontally long feel, so we decided not to perform an epicanthoplasty,
and instead aimed to maintain the line by ensuring a solid ptosis correction in the front area.
The right eye appeared slightly smaller, but after actually dissecting and releasing the eye-opening muscles,
the opening strength of the right eye was better, so the ptosis correction was performed slightly stronger on the left side.
The skin on the front line was also more covered on the left, so the amount of skin removed was greater compared to the right.
Even with such meticulous pre-operative analysis, variables can arise during surgery,
so I believe that flexible and organic judgment is crucial for good results.
Post-Operative Progress


The stitches were removed on the 7th day after surgery.
Even with an incisional double eyelid surgery, if a low line + ptosis correction
is performed securely like in this patient's case, the swelling is not severe.
The eyes open more comfortably, and there is no more looking sleepy due to overlapping skin.
By the 2nd week, the swelling subsided a bit more, looking natural,
and in the 2-month photo, just as the patient wanted, even the hidden double eyelid feel disappeared,
completing a look of monolids with only ptosis correction.
Whether to go for a monolid feel where even the inner double eyelid is covered like now,
or to maintain a slight inner double eyelid depends on the patient's preference,
but the structural conditions of the existing eyes must also be considered.
How much skin excision is needed,
where the line height should be so it doesn't loosen, and the depth of the line must be determined according to skin thickness.
Many men desire similar eye shapes,
but since everyone's conditions are different, it is important to establish a surgical plan through detailed consultation.
