Today, I would like to review a case involving Corner Canthoplasty, Under-eye Fat Repositioning, and Double Chin Liposuction & Muscle Tying.
Before Surgery




The patient is a woman in her late 30s. Although her double chin was not severe,
she felt it was gradually sagging compared to before,
and wanted to undergo surgery preventatively while she had the time.
Since she did not have a lot of fat, I judged that simple liposuction alone would have limited effects,
so I recommended adding a muscle tying procedure.
Because her skin was not severely sagging,
I advised her that a thread lift was not necessary.




For her eyes, she wanted them to appear larger laterally and downward while slightly lowering the outer corners.
Therefore, I recommended Corner Canthoplasty, a method I commonly perform.
For the under-eye area, even without additional fat grafting,
I believed that Under-eye Fat Repositioning alone could sufficiently improve the hollow appearance.
Consultation and Surgical Process
The eye surgery was performed by first doing the Under-eye Fat Repositioning, followed by the Corner Canthoplasty.
Because the hollowness was severe, fat removal was kept to a minimum on the outer side only.
After finishing the procedure by preserving as much fat as possible to properly fill the volume,
the Corner Canthoplasty was performed by anchoring the lower eyelid tarsal plate in an appropriate position,
creating a shape that expands the eyes laterally and downward.
Afterward, under sedation anesthesia, we proceeded with the Double Chin Liposuction and Muscle Tying.
First, fat under the chin is suctioned through a small 2mm incision below the earlobe.
Using this method, the fat above the platysma muscle is initially addressed,
and then a 2cm incision is made along the crease under the chin to perform the muscle tying surgery.
After performing liposuction, the small amount of remaining fat above the muscle is trimmed away,
and a narrow section of the central part of the platysma muscle is partially removed.
Subsequently, both sides of the muscle are dissected so they can be easily gathered to the center,
and they are sutured together in the middle with appropriate tension. This is the general surgical method for double chin muscle tying.
However, there are important additional techniques that must be performed here.
Once the muscle is dissected, the subplatysmal fat located beneath the muscle can be observed.
Excision of these fat tissues is crucial for creating a sharp under-chin contour.
Because this fat is located under the muscle, it cannot be reached with liposuction.
Depending on the patient, for those with a lot of volume here, whether or not this is removed
makes a huge difference in the surgical outcome, so we remove as much of it as possible.
Afterward, the digastric muscle is sometimes additionally tied
or excised, but this was not performed on this patient.
Post-operative Progress

There are no stitches to remove for the Under-eye Fat Repositioning, and for the Corner Canthoplasty, stitches are removed on the 7th day.
For the double chin, a compression dressing is applied immediately after to prevent blood pooling.
This is maintained for one week, and upon removal, the stitches from the under-chin incision are also removed.
In the 1-week photo, slight chemosis (conjunctival swelling) from the Corner Canthoplasty is visible,
but this mostly disappears around the 2-week mark, and you can see that the overall swelling has also subsided significantly.
Because the double chin area is heavily affected by gravity,
until 3-4 weeks post-surgery, when not going out,
wearing a simple facial compression band
is helpful for resolving the residual swelling.

Looking at the 1.5-month photo, swelling under the chin is no longer observed,
and you can confirm a clean jawline has been achieved.
The incision scar is also not very noticeable even when lifting the chin.
If the patient had a more severe double chin, the before and after difference would appear even more dramatic, which shows that
muscle tying is the most effective method for double chins.
