Today, I would like to review a non-implant rhinoplasty case.

First, let's analyze the issues through the patient's pre-operative photos.

Pre-Operative Analysis

​

The patient had a history of undergoing only alar reduction about 4 years ago.

Since she felt reluctant to undergo further nose surgery, she periodically

received filler injections in her glabella and nasal tip until the end of last year.

​

Her original nasal bone structure is not on the lower side but rather high,

and she has a dorsal hump.

The hump is prominent from the 45-degree and lateral views.

Since her nose is overall large and high, there seems to be a limit to how much smaller it can be made.

​

Diagnosis and Surgical Method

Because the original nose is large and high, there is a limit to making it smaller.

We needed to shave the hump and determine the height of the glabella and nasal tip to match that level.

To achieve a curved look, the nasal tip would have to be raised too much, which would make the nose look even larger

and longer, appearing unnatural. Therefore, we decided on a straight-to-slight-curve (jikban) line that is closer to a straight line.

Since the glabella was relatively sunken, we decided to fill that area with artificial dermis.

[OPEN RHINO (SILX)+BULBOUS+HUMP2+LONG+MGD Component humpectomy performed.

Due to concerns about an inverted V deformity and weakening of the keystone area when performing the component humpectomy,

spreader grafts were placed bilaterally using the nasal septum.]

​

To match the lowered nasal bridge height and create a straight-to-slight-curve line, a strut was built using septal extension graft with septal cartilage,

and the nasal tip was elevated using ear cartilage, correcting it to look shorter than the original nose.

The sunken area near the glabella and the rest of the nasal bridge were filled using artificial dermis (Megaderm).

Before Surgery ~ 1 Week Post-Op

Although there is some bruising and swelling around the glabella, the existing hump has been well corrected,

and it has been beautifully corrected into a straight-to-slight-curve line where the nasal tip is only slightly elevated, exactly as intended.

The overall appearance of the nasal tip drooping lengthily after the hump has been improved,

so even though the height has increased, the nose became shorter, balancing the face and giving it a smaller appearance.

2 Weeks ~ 2 Months Post-Op

2 Weeks Post-Op

Compared to the first week, the bruising has largely disappeared,

and the swelling has subsided significantly, making the contours of the nasal bridge and tip more defined.

​

2 Months Post-Op

As the swelling reduced further than before, the glabella that appeared high has settled down,

and the overall contour has become more distinct.

Because the strut was built sturdily, there is no severe drooping or lowering of the nasal tip,

and the scar on the columella has faded significantly.

After about 6 months, the columellar scar is expected to fade even more and become almost invisible.
