Today, we will review a case of natural adhesion and epicanthoplasty.

Pre-operative Analysis
[Before Surgery]

The patient has round, slightly protruding eyes with mild sunken upper eyelids and hidden double eyelids.

Looking at the epicanthal folds, they are developed in a shape that extends down to the lower eyelid area,

causing slight wrinkles visible down to the under-eye aegyo-sal area, and the intercanthal distance is relatively wide at about 40mm.

However, the angle of the epicanthal fold stands almost vertically, so it does not significantly interfere with the front of the double eyelid line,

and the red part called the lacrimal caruncle is also exposed to some extent.

Diagnosis and Surgery

Since the patient already has large, protruding eyes with hidden double eyelids, we determined that there would be no issue creating a double eyelid line using the natural adhesion (non-incisional) method.

Because the patient wanted a natural-looking line that is visible overall rather than a flashy, large double eyelid,

we designed an in-out line raised 3mm from the current line.

The eye-opening muscle strength was good, so ptosis correction was not performed.

Regarding the epicanthoplasty, considering only the intercanthal distance, it is wide enough that we could open up to 2mm on each side, but

1) there is already some exposure of the lacrimal caruncle, and

2) the eye shape is round, so we aimed for a direction that eliminates the stuffy feeling at the front without ruining this image,

and designed it accordingly.

We decided to open horizontally by only 1mm on each side, but perform sufficient internal dissection to

remove the hard, white band-like appearance and

planned a method to eliminate the wrinkles extending down to the aegyo-sal area.

Post-op ~ 2 Months

When natural adhesion surgery is performed, the eyes are greatly affected by the anesthesia injection and swelling on the day of surgery,

so they often appear asymmetrical.

This was the case for this patient as well.

When natural adhesion surgery is performed, the eyes are greatly affected by the anesthesia injection and swelling on the day of surgery,

so they often appear asymmetrical.

This was the case for this patient as well.

However, once the major swelling subsides by the 6th day after surgery, you can see that the symmetry matches well,

and there is an average amount of swelling where the line looks slightly plump.

At about 2 months post-op, the swelling can be considered almost completely resolved.

The line has formed at an appropriate height, and the slight sunken upper eyelid observed before surgery

has also been improved without upper eyelid fat grafting.

In the case of epicanthoplasty, the shape can be seen to change slightly as the scar tissue settles.

Immediately after surgery on the same day, it looks very open, but

by the 1st week when the stitches are removed, it feels like it has closed up again due to the scar tissue.

This scar tissue gradually worsens up to the 1st month,

and then gradually softens after 1 month, undergoing a scar maturation process for about 6 months.

In the 2-month post-op photo, the previously visible wrinkles near the aegyo-sal have improved,

and it is well-settled, being only slightly more open without compromising the round eye image.
