When people think about looking older, most only consider lifting~
It's true that lifting is the most key point.
However, as a plastic surgeon,
I'd like to talk about hollowness...🙆♂️

Why does facial hollowness make you look older?

If you compare photos of someone in their 20s and 40s,
✔️The temples are hollow
✔️The cheeks next to the cheekbones are hollow
✔️The area above the eyes is hollow
✔️The area under the eyes is also slightly hollow
These hollow areas make one look very tired
and give an older impression.
If you look at the biggest difference between children and older adults,
how does the face change as one ages?

This skull-like shape, or contour, becomes more pronounced
in this way.
Do you see the facial contours when you look at a child's face?
No, you don't, right?

Filling in those hollow areas
is just as important as lifting, in my opinion.
How to solve facial hollowness?

The way to solve it is, in fact,
to fill in the hollow areas.

There are largely three ways to add volume to those hollow areas:
① Fillers
② Fat grafting
③ Collagen boosters
So, which areas are best for fillers, fat grafting, or collagen boosters?


This patient has a slightly uneven and hollow area from the forehead to the temples.
In such cases, fat grafting is generally recommended.
The primary reason is the larger volume needed.
Even if you try to use filler for the entire forehead and temples, more than 10cc would be required.
Because fillers have a very soft consistency,
if you inject 10cc or more into the forehead,
the tissue won't be fixed.

It can sag and feel like slime when pressed,
easily indenting.


Fillers are recommended only for a part of the temples or forehead,
but for overall volume and to address more significant hollowness,
fat grafting is a better option.

Therefore, in the case of the model above,
fat grafting was performed to address the overall hollowness of the forehead and temples.

Another model also had fat grafting on the forehead and temples,
and you can see a significant before-and-after difference, making them look much younger and their face appear rounder, right?

This person's forehead and temples were treated with fillers.
Earlier, I mentioned that for filling the entire hollow areas of the forehead and temples,
I recommend fat grafting more.
However, in this person's case, the upper forehead was already quite prominent, like a 'Jjanggu' cartoon character,
and a large amount of volume didn't seem necessary.
Even a slight fill could yield significant results,
so I only added a small amount of filler.

The volume significantly improved, making the narrower forehead appear wider,
and giving the overall image a brighter feel.
Therefore, in such cases,
I believe fillers can also achieve great results.

This person stated they didn't want their forehead to protrude further,
and due to the temple and cheekbone areas,
their overall face shape was indented like an hourglass.
They also felt their face looked long and tired.

This is how it was resolved with temple fillers.
The face looks much softer than before the procedure, right?
It also gives a more youthful impression.

As one ages,
hollowing of the side cheeks becomes more common.
Some people have hollow side cheeks due to large cheekbones, but for them,
it would be best to reduce the cheekbones while filling the side cheeks.
However, even in other cases, filling the temples a bit more
can give the face a much softer appearance.

In this case, the side cheeks were filled with a collagen booster.
I actually prefer collagen boosters for filling the side cheeks.

The reason is that when fillers are injected, they don't lift well.
For example, the base needs to be firm
to push it up,

but because the base isn't firm and doesn't provide support,
it has the characteristic of not lifting well even if you want it to.
That's why when you get fillers,
it might look good at first but then sink back in,
which can be a bit disappointing later.

However, collagen boosters
stimulate the tissue to produce collagen itself,
so as time passes, collagen gradually builds up,
creating volume in the face, which is why I prefer them.
What about fat grafting?
Occasionally, the skin is too tight,
and fat doesn't engraft well.
In fact, the side cheeks are one of the areas where engraftment is most difficult.
The tissue is too tight, so even if fat is injected,
it keeps failing to engraft and sinks back in...

In such cases, I recommend collagen boosters.


This is a case where the side cheeks were filled with a small amount of filler due to hollowing.
It's a case where only a small amount was injected,
but the resulting volume made the face much softer than before.
In conclusion, if a large amount is needed for one area,
I tend to avoid fillers a bit more.
If only a small amount is needed, fillers are actually the most cost-effective.
They're inexpensive, and if you don't like them later, they can be dissolved and removed.

It seems that for some areas, collagen boosters are better,
and for others, fillers are better.
What are the types of fillers and collagen boosters?

Foreign filler products are
Juvéderm and Restylane.

Among domestic fillers, Chaeum and Neuramis.

Collagen boosters can be broadly divided into two types.
