Today, let's take a look at an actual patient who underwent incisional double eyelid surgery at our clinic, along with their photos.

A teenage female patient visited our clinic with her mother

after being recommended by a friend who had surgery here.

Pre-operative Analysis

Patient's Pre-operative Condition

✅ Narrow horizontal width

✅ Wide distance between the eyes (glabella)

✅ Asymmetrical eyebrow height

She had monolids with a narrow horizontal width,

and the wide distance between her eyes gave a somewhat stuffy impression.

Additionally, the height of her eyebrows was different,

and the degree of eyelid sagging varied, resulting in asymmetrical eyes.

Desired Eye Shape

The patient wanted a clear, visible double eyelid line from the front,

and expressed a desire for larger, more defined eyes.

Diagnosis and Surgery

✅ Thick eyelid skin (but not much fat above the eyes)

✅ Deep-set eyes

This patient has thick eyelid skin and an eye structure closer to deep-set eyes rather than protruding eyes.

To create a large and distinct double eyelid line, the incisional double eyelid method seemed necessary.

We also determined that performing an epicanthoplasty together would result in a harmonious eye and double eyelid shape.

Although the skin is on the thicker side, there isn't a bulky amount of fat above the eyes.

However, because there is a difference in the position of both eyebrows and the degree of eyelid sagging,

the amount of skin excision on each side must be different to create identical double eyelids.

Since there was no ptosis, ptosis correction was not performed,

and lateral canthoplasty was not done because there was little space at the outer corners of the eyes to extend.

As a result, we proceeded with incisional double eyelid surgery and epicanthoplasty.

Post-operative Progress

6 Days Post-Op

This is the appearance after stitch removal on the 6th day post-op.

There is still some swelling, but both double eyelid lines have been created symmetrically and beautifully.

2 Months Post-Op

This is the appearance with all the swelling gone.

With a distinct semi-out fold, her eyes now give a large and refreshing impression.

Although it is a relatively large double eyelid, the swelling below the line has completely subsided,

revealing the inner fold and crease lines, showing a very natural look.

The depth of the double eyelid is also just right—neither too deep nor too shallow—

and the scar from the incisional surgery is not prominent when closing the eyes or looking down.

However, there is slight redness along the scar line due to skin pigmentation,

but this disappears in the 6-month post-op photos.

6 Months Post-Op

In the 2-month photos, the skin looked slightly red along the scar line,

but you can see that it has completely faded in these 6-month photos.

As a result, with natural yet large and refreshing double eyelids

and clean surgical marks, the patient was highly satisfied.
